Employee removed an inspection plate cover just below the airlock feeder on the cyclone located on Silo #3. The airlock feeder was running; employee's right hand contacted the airlock rotor, cutting off tips of the index, middle and ring fingers. Employee was treated at local ER.

WET ROAD CONDITIONS. DRIVER LOST CONTROL OF TRACTOR IN CURVE ON HWY 136 EAST, PICKENS CO., GA. STRUCK ANOTHER TRUCK AND WENT DOWN AN EMBANKMENT. CAB CAME LOOSE FROM FRAME AND LANDED ON BOTTOM OF A STEEP BANK. SPEED WAS NOT A FACTOR JUST WET ROAD CONDITIONS ON PAVEMENT WHICH LEAD TO A LOSS OF CONTROL. DRIVER RECEIVED A BROKEN LEG, TWO BROKEN RIBS AND A CUT ON FOREHEAD.

TAKING SAMPLE FROM A BULK TRUCK JUST LOADED. WHEN THE CABLE HOLDING THE LOADING SPOUT IN THE RETRACTED POSITION BROKE.THIS ALLOWED THE SPOUT TO FREE-FALL ONTO EMPL HAND BEFORE EMPL COULD MOVE OUT OF THE WAY.
