Colorado Materials, Ltd · Over-exertion in lifting objects

Employee was working maintenance on a conveyor belt system and felt a strain in EE's lower back while EE lifted the belt. Upon being interviewed, EE stated that EE believed EE aggravated a prior non-work related injury that had put EE out of work for several months.

While performing mechanic duties, HOLT employee and Weisman Equipment contractor, was embedded in the left chest area by a small piece of metal shard from a piece of equipment. The employee was taken to a medical facility to receive medical treatment and the foreign body was removed from the chest.

Employee was removing a carrier roller and support block from a 336 Caterpillar Machine. To remove the block the employee was using a metal punch and hammer. When the hammer stuck the punch a shard of metal chipped off striking the employee's chest.

Employee was guiding a forklift moving a heavy motor. the motor became lodged on the floor of a connex, and in attempting to dislodge the motor, it rolled on the top of their right foot.

Conducting pressure checks on a brake valve, EE went between the right front tire and the frame to attach the pressure gauge. The other mechanic in the cab turned the wheel to the left, thinking they would check the valves in the same order as the previous truck, and this trapped employee between the tire and the frame. EE was diagnosed with four broken ribs and a punctured lung.

EE was assisting in opening the lid of the ISC 103 crusher when EE's sleeve caught on the machine and EE was unable to move their hand out of the slot EE was pushing from as the lid was pushed back. EE's hand was bent backwards until EE was able to pull it free, but EE's palm was cut as a result of being caught.

EE was removing a head pulley bearing at 103 VSI plant. After removing the bearing housing and shaft sleeve, EE was hitting it out with short handle anvil hammer. While tapping it out, a piece of shrapnel from the hammer struck EE, penetrating EE's glove and right index finger. This caused a laceration and fracture of EE's finger.

EE was on the ground cleaning spillage from the overland conveyor transfer chute when EE stepped on a loose rock and twisted ankle. Diagnosed as a sprained ankle.

Supervisor was using a hammer to remove bearings for the wash plant shaker screen, when the hammer struck a bearing directly. The bearing shattered and some of the splintered pieces struck EE as EE was standing approximately 12' behind the supervisor. One piece struck EE's chin and another embedded in right forearm.

Employee was feeding material into portable screening plant. The plant shut off so the employee went to check plant for problems. EE attempted to remove the radiator cap from the plants engine. As EE turned the cap, it blew off and hot liquid from the radiator splashed onto left forearm burning EE.
