Cahaba Resources, LLC · Accident type, without injuries

A blaster for sub-contractor, Austin Powder, left his truck in reverse gear and let truck roll off 14-16'high embankment. No injuries, removed truck and drove out of mine site, attached to wrecker, and went to Austin Powder shop.

Operator continued to back up on existing berm, and eventually backed over berm, truck overturned. Operator injured hand while in truck when overturning.

HD 325 truck being loaded by PC 1100, operator had arm out of window. When rock fell into bed of truck, truck door jarred operator's arm and shoulder.

J&MN, Inc. · Struck against a moving object

Driving a 50 ton rock truck when EE drove onto the soft shoulder of the road causing the road to give away & the truck to overturn. EE was wearing the seatbelt. This was a new road & berm had not been installed.
