Associate was dismounting equipment and to avoid stepping on a pile of mud he attempted to step over it and his foot gave way and twisted causing a stress fracture to his right foot.

Miner stated he stooped down while talking and lost his balance. He reached out to catch himself and grabbed a ramp for loading equipment and the ramp fell hitting him in the forehead. Miner received 4 stitches to the upper forehead.

Miner was helping another miner to pick up an irrigation pipe and move it to the side when he felt a pin in his lower back. The footing was loose and his foot slip and caused additonal pressure to one side. Strained lower back area.

Associate was unbolting a roller from a scraper without proper support and when bolt was removed the roller twisted and pinch the associate's pinky resulting in it being smashed and lacerated.

Employee was unloading scrapper teeth from a loader bucket that was lowered to the ground, and neatly stacking the teeth on a trailer. The employee was carrying one tooth (approximate weight 12.5 pounds each) in each hand, while setting down the tooth with the left hand, employee felt a "pop" in his left arm, that is when he noticed a knot on his arm and felt pain.

The miner walked down access steps from the Mining Service Trailer. His right foot slipped on the last step causing him to lose his balance. The miner suffered strains to his right hip and lower back. There was mud on the bottom of the miner's boots, the last step tilted downward approximately 5 degrees, and there was smooth metal framing the step.

Employee was entering mining break room. As he pulled the door closed from inside with his left hand, he was touching the door with his right hand. The hinged edge of the door caught the middle finger of his right hand, ee received a severe avulsion.
