EE was installing a new chain sprocket on the digger drive. After the completion of the project, EE felt pain in his right hip. Plant manager was not notified of injury until 3/15/2006. EE was seen by Dr. & recommended to have MRI and be seen by surgeon. EE underwent surgery on 3/21/2005 for herniated disc.

EMPLOYER WAS USING ACETYLENE TORCH WHEN SPARKS CAUGHT EE'S INSULATED COVERALLS ON FIRE. HE RECEIVED 2ND DEGREE BURNS TO CALVE OF RIGHT LEG. INITIAL TREATMENT WAS FIRST AID BUT EE LATER DEVELOP ED AN INFECTION THAT REQUIRED HOSPITALIZATION TO TREAT WITH ANTIBIOTICS.

54736YEE STATED THAT OPERATING FRONT-END LOADER AROUND PLANT LOADING TRUCKS CAUSED HIM TO HAVE LOW BACK PAIN. HE BEGAN PHYSICAL THERAPY ON 8-11-92 WHICH TURNED THE CLAIM INTO A MEDICAL TREATME 53031SE.
