WHILE REPLACING & TIMING A SET OF TAPER LOCKING DRIVE GEARS ON COUNTER WEIGHTED SHAFTS, ONE GEAR DISENGAGED FROM TAPER LOCK HUB ALLOWING OPPOSITE GEAR & SHAFT TO FREEWHEEL TO ITS RELAXED POSIT ION. VICTIM HAD HIS RIGHT HAND ON ONE OF THE GEARS, HIS RIGHT THUMB TIP WAS PULLED THROUGH THE GEARS.

WET GROUND CONDITIONS. OPERATOR WAS CLIMBING LADDER WHEN HIS LEFT HAND SLIPPED OFF HANDRAIL, CAUSING HIS FEET TO SLIP OFF STEPS. HE WAS ABLETO CATCH HIMSELF WITH HIS RT HAND; DURING FALL, HIS BODY TWISTED, CAUSING MUSCLES IN HIS STOMACHTO BE PULLED HARD ENOUGH TO STRAIN THEM.

EE WAS SHARPENIG A PICK AX, AX POINT WAS TOUCHEDTO WHEEL AT THE WRONG ANGLE CAUSING WHEEL TO GRAB AX POINT. THE FORCE OF THE POINT HITTING THE GUARD WAS ENOUGHT TO BREAK THE GUARD WHICH ALLOW ED THE POINT TO BE PULLED INTO THE WHEEL HOUSINGSOONER THAN EE COULD RELEASE HIS GRIP
