Continuous miner was pulling last rib on block when a roof fall occurred causing continuous miner to be trapped under rock fall.

Shuttle car was stuck on feeder scoop, was lifting shuttle car. Injured was hooking up a chain to shuttle car; shuttle car slipped off scoop bucket causing bucket to jar, knocking EE to rib.

Injured was sitting w/intersection bolting miner. Shuttle car trammed into but ran over rock causing rock to fly striking injured in the forehead.

Injured was pulling the contactor case from Eimco shuttle car. He felt pain in back, he finished shift and tried to work the next day. Pain was too bad and he had to go to doctor.

Working place #6 entry cut back into 10 rm causing a three way intersection. The breakthru at 10 rm fell. 4 ft high, 17 ft wide and 20 ft long.

Injured was helping to install a 4 ft roof bolt, bolt started to bend, he put hand up to straighten bolt. Co-worker put up pressure on bolt causing it to bend and caught injured's right thumb against roof.

INJURED RAISED HIMSELF OUT OF CANOPY WITH HA ND ON STEERING PIVOT STOP TO SEE LOCATION OF BOLTER CABLE WHEN KNEE PUSHED STEERING LEVER CAUSING SCOOP TO PIVOT AND SMASH LEFT RING FINGER.
