A ROOF FALL WAS DISCOVERED INTHE #2 ENTRY 3 CROSS CUTS INBY THEPORTAL. THE ROOF FALL IS APPROX 30' LONG X 20' W X 6-8' HIGH. THE FALL WIL BE CLEANED UP AND RESUPPORTED WITH CRIBS AND CROSS C OLLERS.

EE STATED THAT HE WAS HUALING BOLT STRUCTURE. WHILE LOADING THE STRUCTURE ONTO THE SCOOP HE LIFTED A PIECE TO LOSS ONTO THE SCOOP AND TWISTED HIS LEFT KNEE. HE CONTINUED TO WORK UNTIL 12-13-93 WHEN HE HAD SURGERY.

HE STATED THAT HE WAS GREASING THE MINER AT SERVICING TIME WHILE THE MINER CREW WAS GETTING A ROCK OFF THE TOP OF THE MINER.AS THE ROCK WAS BEING BEATEN WITH A SLEDGE HAMMER, A SMALL PIECE FLE W APPROX. 5' AND HIT HIM IN THE LEFT EYE.
HE STATED THAT HE WAS OPERATING A SHUTTLE CAR ON 002 SECITON WHEN HE RAN OVER A PIECE OF ROCK THAT WAS LIEING IN THE ROADWAY. HE WAS THROWN UP AND STRUCK HIS HEAD ON THE CANOPY OF THE SHUTTLE CAR.

THE HANGER REMOVED IN PLACE AND REOUTBY EE BREAKINGHIS FALL BY GRABBING THE HANGER HIS LEFT SHOULDER WAS DISLOCATED.

DURING REGULAR INSP OF THE RETURN A ROOF FALL WAS DISCOVERED.LOCATION OF FALL IS IN #2 ENTRY OF 001 SECTION RETURN FROM BRK #27 OUTBY CONNECTING TO ROOF FALL REPORTED TO MSHA ON 5-7-90.SIZE OF FALL 11'HIGH X 20'WIDE X 20'LONG.FALL HAD NO EFFECT ON PRODUCTION OR VENTILATION.
HE STATED HE WAS MOVING CURTAIN FROM THE PATH OF THE MIENR WHEN HIS FOOT SHIPPED AND TWISTED HIS RIGHT KNEE. HE CONTINUED TOW ORK THE REST OF THE SHIFT AND REPORTED THE ACCIDENT TO THE MINE S UPERINTENDANT. THIS WAS NOT A MSHA REPORTABLE ACCIDENT WITH 1-14-91 WHEN IT BECAME A LOSS TIME ACCIDENT.

I.NAME LIFTING MINER CABLE OUT OF HANGERS ON MINER,FELT SHARP PAIN IN LOWER BACK.CONTINUED TO WORK REST OF SHIFT,CONSULTED DR NEXT DAY,TOLD BY DR POSSIBLE REINJURED FROM 3-2-90.
INJURED STATED THAT HE WAS SITTING IN THE SEAT OF THE STANDARD SHUTTLE CAR AND HE TWISTED AROUND TO CHANGE DIRECTION. HE STATED THAT WHEN HE TWISTED HE FELT A SHARP PAIN IN HIS LOWER BACK.
INJURED STATED THAT HE WAS TRYING TO FREE A PIECE OF DRILL STEEL FROM A DRILLED HOLE IN THE MINE ROOF. THE DRILL STEEL FELL OUT OF THE HOLE AND STRUCK HIM ON THE RIGHT HAND THE SECOND KNUCKLE .
EE STATED THAT WHILE HE WAS ATTEMPTING TO REMOVE DRILL STEEL FROM THE HOLE IN THE ROOF A PIECE OF DRAW ROCK APPROX. 12"X12"X3" IN THICKNESS BELT ON HIS LEFT HAND WHICH WAS RESTING ON THE DRILL HEAD OF THE ROOF BOLTER THE ROCK BROKE HIS LEFT INDEX FINGER.
I.NAME STATED THAT HE WAS MOVING CABLE OUT OF THE ROADWAY WHEN HE FELL AND STRUCK HIS KNEE ON THE BOTTOM OF THE MINE.
A ROOF FALL WAS DISCOVERED ON THE WEEKLY EXAMINATION. THE ROOF FALL IS LOCATED IN THE NO.1 RETURN OF THE FIRST BELT APPROX. 16 OPEN BREAKS INBY THE OVERCAST LEADING TO 002 SECTION. THE ROOF FA LL IS APPROX. 40' LONG X 20' WIDE. IT HAS ALSO FELL FOR ABOUT 20' INTO #2 RIGHT. THE HEIGHT OF THE ROOF FALL IS ABOUT 5 FEET. TIMBERS AND CRIBS ARE TO BE INSTALLED IN THE ADJACENT AREA.
HE STATED THAT HE PICKED UP A TIMBER AND WAS TWISTING AROUND TO SET IT WITH HE FELT A SHARP PAIN IN HIS LOWER BACK. HE WAS TRANSPORTED FROM THE MINE TO THE HOSPITAL AT THAT TIME.
I.NAME WAS LIFTING A DRIVE LINE CONTINUOUS MINER, WHEN HE FELT A PAIN BETWEEN HIS SHOULDERS. I.NAME CONTINUED TO WORK THE REST OF THE SHIFT AND 6 1/2 HOURS ON THE 27TH OF AUGUST. HE CONSULTED A DOCTOR ON 8/28/90 AND RETURNED TO WORK ON 8/30/90.
HE STATED THAT HE WAS REMOVING ROCK DUST FROM THE #2 BELT LINE. HE STATED THAT HE DIDN'T FEEL ANY PAIN AT THE TIME OF THE ACCIDENT BUT DID THE FOLLOWING MORNING IN HIS LOWER BACK.
HE STATED THAT HE WAS ROLLING A WHEEL UNTIT INTO THE SCOOP BUCKET WHEN HE FELT PAIN IN THE LOWER LEFT SIDE OF HIS BACK. HE CONTINUED TO WORK UNTIL 8-20-90 WHEN HE CONSULTED A DOCTOR AND MISSED THE FIRST SHIFT AFTER THE ACCIDENT.
I.NAME MOVING SCOOP BATTERY STAND TO LEVEL IT UP ,FELT PAIN IN LOWER BACK.CONTINUED TO WORK UNTIL 8-15 THEN CONSULTED DR.
I.NAME GOT OFF SHUTTLE CAR,IN LAST OPEN X-CUT OF 002 SECTION BETWEEN #1 & #2 HEADINGS WHEN PIECE OF MINE ROOF 3' X 4' FROM 1-18" THICK FELL FROM BETWEEN ROOF BOLTS,STRUCK I.NAME MIDWAY OF BACK AND UPPER BODY.
EE WAS CROSS A STOPPED OCNVEYOR BELT ON 002 SECTION WHEN HE FELT PAIN IN HIS LOWER BACK. HE CONTINUED TO WORK FOR 2 SHIFTS ON SAT AUG 4 HE PAIN HAD INCREASE SO HE CONSULTED A DR ON 8-4-90.
EE STATED THAT GOT COAL IN HIS KNEE PAD. THE COAL CUT HIS KNEE AND BECAME INFECTED HE REPORTED THIS ACCIDENT ON 8-1-90 HE CONTINUED TO WORK UNTIL AUGUST 8, 1990 WHEN HE CONSULTED A DR.
I.NAME HELPING RAIL RUNNER OPER UNLOAD MINER TRAM MOTOR FROM RAIL RUNNER.TRAM MOTOR ROLLED OVER AND CAUGHT I.NAME RIGHT WRIST BETWEEN TRAM MOTOR AND RAIL RUNNER.
EMPL STATED THAT HE WAS ROOF BOLTING ON 001 SECTION WHEN ACCIDENT HAPPENED. HE STATED PUT BENT6 ROOF BOLT INTO HOLE IN ROOF & ATTEMPTED TO STRAIGHTEN BOLT. EMPL STATED THAT WHEN HE PUSHED AGAI NST ROOF BOLT RIGHT ELBOW BENT WRONG DIRECTION. THIS WAS NOT AN MSHA REPORTABLE ACCIDENT UNTIL EMPL MISSED WORK ON 9/12 DUE TO ELBOW. EMPL HAD VISITED DR ON 8/30 BUT WASN'T LOST TIME.

HE FINISHED AND WALKED TO THE #2 ENTRY WHERE THE MIN ER WAS CUTTING COAL. HE WAITED FOR THE STD. SHUTTLE CAR TO GO BY WHICH WAS HAULING IN THE #2 ENTRY BUT AS THE CAR PASSED UNDER THE ROOF BOLTER CABLE IT WAS CAUGHT BY THE CAR AND PULLED DOWN F
EMPL STATED THAT HE WAS OPERATING LEFT SIDE OF ROOF BOLTER WHEN PIECE OF LOOSE ROOF MATERIAL FELL & STRUCK HIM ON LEFT SIDE OF HEAD & EAR.
HE STATED THAT THE MINER WS CUTTING A LEFT BREAK AND COAL COME FROM THE MIENR TAIL AND STRUCK HIM IN THE RIGHT EYE. HE WAS OPERATING THE STANDARD CAR AT THE TIME OF THE ACCIDENT.

I.NAME STATED THAT HE WAS HANGING HIGH VOLTAGE CABLE WITH METAL INSULATING HANGERS FROM ROOF BOLT PLATES. I.NAME STATED THAT HE HUNG THE HANGER IN THE PLATE AND WAS ATTEMPTING TO FASTEN THE HA NGER AROUND THE CABLE WHEN THE CABLE AND HANGER FELL AND CAUGHT I.NAME'S FINGER AS IT FELL TO THE GROUND.
APPROX 6:20 AM 5/10 ROOF FALL WAS DISCOVERED BY DAY SHIFTMAN TRIP. LOCATION OF FALL WAS AT 2ND BREAK ON FIRST RIGHT ON TRACK ENTRY. FALL WAS APPROX 8' THICK X 20' LONG X 18' WIDE. AREA WAS CRI BBED DANGERED OFF. ROCK WAS MOVED WITH SCOOP & GOBBED. ADDITIONAL SUPPORT WAS PROVIDED BY MORE CRIBS & CROSSCOLLERS.
APPROX 5:45 AM 5/7 ROOF FALL WAS DISCOVERED ON PRESHIFT EXAMINATION. LOCATION OF FALL WAS IN #2 ENTRY OF 002 SECTION BETWEEN 25TH & 26TH LINE OF BREAKS. FALL WAS APPROX 8' THICK X 35' LONG X 2 0' IN WIDTH. CRIBS WERE SET IN FALL AREA & AREA WAS DANGERED OFF. ROOF FALL HAD NO AFFECT ON VENTILATION NOR PRODUCTION.
TRAMMING S CAR, HIT POTHOLE ON ROAD CAR WENT IN HOLE HE WENT IN CANOPY OF S CAR
EMPL STATED THAT HE PUT FIRST ROOFBOLT NEXT TO RIB & WAS WAITING FOR OTHER OPERATOR TO FINISH PUTTING BOLT UP WHEN PIECE OF DRAW ROCK APPROX 1 1/2' LONGX6" WIDEX3" MAXIMUM THICKNESS FELL FROM ROOF. ROCK STRUCK RIGHT ARM & LEG.
HE STATED THAT HE HAD THE BOLT MACHINE HEAD SWUNG OUT TO THE RIGHT SIDE AND NEEDED TO PULL THE BOLT MACHINE FORWARD WHEN HE TRAMMED ROOFBOLT ON THE RACK OF THE BOLT MACHINE CAME INTO CONTACT W ITH THE TIRE CAUSING THE ROOF BOLT TO FLY OUT OF THE RACK AND STRIKE HIM.
EE OPERATING THE ROOF BOLTER ON 001 SECTION WHEN HE STRUCK HIS RIGHT ELBOW ON THE BOLT MACHINE AS HE PULLED A ROOF BOLT FROM THE RACK. HE REPORTED THIS ACCIDENT ON THE DAY OF THE ACCIDENT HOWE VER IT WAS NOT MSHA REPORTABLE UNTIL AUGUST 2 WHEN HE WAS ABSENT FROM WORK TO CONSULT A DR.
EE STATED THAT HE WAS DRIVING A BEARING ON TO S A SHAFT ON THE TAILPIECE WITH A 12 LB SLEDGE HAMMER AND FELT PAIN IN HIS LEFT ELBOW AFTERWARD. THIS WAS NOT A REPORTABLE ACCIDENT UNTIL 4-23-90 WHEN EE CONSULTED A PHYSICIAN.
HE STATED THAT HE AND ANOTHER EMPLOYEE WERE ATTEMPTING TO INSTALL HOPPERS ON A FEEDER. HE WAS LIFTING ONE END OF THE FEEDER HOPPER WHLE OTHER EMPLOYEE WAS PUSHING THE OTHER END. THE HOPPER S LID FORWARD CATCHING FINGER.
