Victim was last observed accessing the stern if barge NBN-611. Shortly after, he was noticed to be missing. Snow was present on the barge. Damage or impairment is unknown at this time.

Employee cut by sharp metal pipe. The crew was installing a safety mid-rail on a walkway when the associate was struck by the sharp end of a 1 1/4 pipe. The Pipe contacted the right hand between the thumb and index finger causing a laceration the injury required other than first aid.

Associate was attempting to rotate a spud collar when EE lost EE's grip causing the associate to lose EE's balance a fall to the deck from ground Level.

The crew was changing an anchor wire on the Dredge IV when the load shifted unexpectedly. The shift causing the lifting equipment to become taut thus causing a recoil action on the chain. The associate was struck in the lower portion of the facial area. The associate received a small cut on the left cheek and lower lip.

EE was attempting to remove a line used to secure barge on dredge. The line had become fouled during normal operation. EE attempted to jerk the line free with right hand when the injury happened in the right shoulder area. Management was notified and arrangements were made for a medical evaluation.

While attempting to dislodge a short length of barge rope impeding the dredge pump, the victim's hand slipped, and EE sustained a cut to left middle finger when it came against the pump wear plate edge. The resulting laceration required three sutures, and the employee was placed on restricted duty until 07/18/2016 to keep injury clean and dry.

Victim was performing normal tasks (hosing decks, positioning barges, testing sand samples, etc.) When he began to experience lower back discomfort.

While using a box cutter knife to cut the long ends of plastic wire ties, the knife slipped resulting in a 2 cm laceration on the top of the victim's left hand. The wound was closed by a physician with two stitches.

After obtaining a sand sample from a barge being loaded at the dredge, the victim stepped down to the dredge deck (a distance of 12" to 15" down). His left knee "popped" as his weight made contact with the lower level of the dredge.

Employee was engaged in the task of securing a loaded aggregate barge to an empty barge along side the dredge. In the process of placing the 2" diameter soft line up on the deck fitting of the empty barge, the victim says that he strained his back muscle.

EE was moving barge rigging rachets (approx. weight 50 lbs.)from the dredge tender vessel to the barge deck. As a result of this action, EE reported that he sustained a muscle strain in his lower back.

EE lifted and tossed the "eye" end of a 5/8" diameter cable that was attached to a barge drop-back winch on the dredge. The cable end that he was tossing over the barge deck fitting was estimated at less than 20-pounds for the portion that he held. The EE claimed that his elbow "popped" as he tossed the eye end.

While wiping down the inside walls of the dredge classifier, the EE was sitting on a plank with his feet on the floor. He leaned out to clean the walls surface and his hand (holding a cloth) slipped causing him to fall forward. The over-extension allegedly led to a muscle strain.

While in the process of releasing the tension on a fixed mounted barge winch on barge hmt-408, the ratchet handle of the winch "kicked" toward the employee. The sudden movement of the handle caused a muscle strain in the employee's right shoulder. The employee visited an emergency room where he was given a muscle relaxer.

While stepping from the fore-deck of the M/V Delta Kelly J onto the river bank, the employee injured his right knee. It was a re-injury to an initial knee problem sustained while the employee was serving in the military service.

While the employee was walking along the walkway on the side of a barge, he had his arm draped inside the cargo box. He rubbed his arm along a rough area of the inside of the barge cargo box coaming and sustained a minor cut on his left forearm. He was given a tetanus shot and a flake of rust was removed.

Employee was climbing a fixed position ladder when he sustained a spider bite to his lower left arem. The employee had not missed work but visited a doctor who has prescribed a tetnus injection, an allergy injection, and a regiment of six penicillin shots. It is believed that the bite was from a brown recluse spider.

Employee purports to injuring his back while pushing on a 14" dia. pipe while in a seated position pushing with his feet. He was engaged in a maintenance project with others on the re-assembly of the dredge suction line.

EE WAS PUTTING ON A PAIR OF GLOVES. APPARENTLY A SMALL SPIDER (POSSIBLY A BROWN RECLUSE SPIDER)WAS IN THE FINGER OF ONE OF THE GLOVES. THE EE WAS BITTEN ON THE LEFT RING FINGER AND AGAIN ON HIS LEFT FOREARM AS HE WAS QUICKLY REMOVING THE GLOVE.

EMPLOYEE WAS ASSISTING IN POSITIONING A DECK WINCH. USING PRYBARS, TWO MEN WERE MOVING THE WINCH INTO POSITION WHEN ONE BAR SLIPPED. THE LOAD (APPROXIMATELY 250 POUNDS) WAS TRANSFERRED TO THE OTHER BAR. THE TRANSFER OF WEIGHT PINNED THE INJURED EMPLOYEE'S FINGER BETWEEN THE PRY BAR AND AN ELECTRIC MOTOR.

EE WAS ATTEMPTING TO MAKE A CUT WITH A HACKSAW ON A DEFECTIVE BEARING. THE UNSECURED BEARING MOVED IN THE PROCESS AND THE BLADE OF THE SIMPLEMANUAL- OPERATED HACKSAW CUT THE SECOND FINGER OF THE EE'S LT. HAND.

WHILE POSITIONING A SPUD ON THE DREDGE, THE EMPLOYER WAS REMOVING A PIN. THE LIFTING COLLAR WAS NOT PROPERLY SECURED AND DROPPED ABOUT 12" CATCHING HIS RIGHT HAND MIDDLE FINGER BETWEEN THE PIN HE WAS REMOVING AND THE COLLAR ON THE SPUD. THE COLLAR SHOULD HAVE BEEN PROPERLY SECURED BEFORE ATTEMPTING TO REMOVE THE PIN.

SOMETIME DURING THE SHIFT EE SUSTAINED A WELDING FLASH BURN TO HIS EYES EITHER FROM CATCHING A DIRECT FLASH OR FROM A REFLECTION OFF THE WATER. HE WAS WEARING SAFETY GLASSES AND INTERMITTENTLY A FACE SHIELD OR WELDING HOOD.
