ee was assisting with a belt pull. When pushing on the belt, felt a pop in EE's knee. was evaluated and was put off work.

ee was climbing up stairs; reached out to grab the handrail and felt pain in middle back area.

after drawing out feeder chute and running it dry, ee locked out belt and feeder and crawled up into chute to investigate a potential obstruction. After poking around with a pry bar, material from the overhead stockpile sluffed off and engulfed ee in the chute. EE was extracted and is recovering at home (lacerations and contusions)

employee was using a long pry bar to hit the side of the chute to help clear the built up stone. EE was twisting while struck the chute and felt a sharp pain in EE's lower back.

ee was moving a diesel pump in the quarry. ee picked up the suction hose and felt a strain in their back.

EE was walking across the parking lot when EE slipped on the ice and fell. EE's face made contact with the ground, causing EE's safety glasses to push into EE's forehead resulting in a small laceration above EE's eye that required 4 stitches. No restrictions issued- cleared to RTW.

EE was using a torch to cut the cutting edge on a loader, a large piece of it fell and landed on EE's left foot resulting in a fracture.

EE picked up a large stone to remove it from the roadway. As EE picked it up, it broke into pieces. A sharp edge of the stone cut the ee's finger. Laceration requires stitches. No further restrictions or treatment administered.

EE lost control of haul truck & veered off haul road. Operator sustained minor bruising from seatbelt. Cause of incident under investigation - weather conditions unfavorable at the time.

On 2-1-24 EE reported to EE's foreman that on the previous work day1-31-24, EE's buttocks were burned to the point of blisters. EE said these were caused by the seat warmer. When asked why EE did not turn off the seat warmer, EE replied, "I did not think it was that hot"

Injured was returning to the quarry with unloaded Cat tractor/trailer haul truck. Injured was momentarily distracted by an unidentified object on the left side of the haul road which caused EE to drift toward the rock berm on the right side. The right rear trailer tires struck a rock which caused EE to lose control and exit the road.

A team of 4 employees was putting a pulley on #1 scalper. When ratcheting on a chain fall, the pulley slid/moved. Injured was not directly engaged with the pulley, but jumped backward when the pulley shifted and jarred EE's back.

Injured was using a bar to clear a chute at the 21 ft. crusher when EE felt pain in the wrist. Injured did not report the incident until 08/31 when EE requested medical treatment.

Employee states that as EE was walking from EE's vehicle to the office to clock in, EE tripped over a piece of wood sticking out of the ground from a sign post that had previously broken. As EE fell, EE states that EE landed on EE's left arm

Employee was accessing a haul truck by means of a cable type 3 step ladder when EE's foot slipped off the step and EE took the brunt of EE's weight with EE's left arm resulting with a strain to upper left arm.

Miner was performing clean-up activities using a water hose in a limited headroom area. As the miner was washing, EE hit head which caused EE to fall to the deck and breaking left leg, requiring surgery.

Employee suffered muscle strain and severe bruising after steering shaft broke on 768C haul truck. Broken shaft ripped supplemental steering harness out as well, leaving driver only able to hold on as truck ran off haul road into drainage ditch. Truck stayed on wheels at all times.

Employee was attempting to clear some material that had rolled under the trailer wheels of the truck and could not be reached with the equipment at hand. As ee exerted some force against the rock, ee foot slipped on some loose material causing ee to over exert pressure resulting in an injury to right thumb area

Employee was operating a hand flow gate in boat loading tunnel. Gate was stiff and employee felt a sharp pain in left shoulder.

Injured was greasing bearing on #2 conveyor top catwalk when the board he was walking on broke.

Injured was chipping slag after welding on a haul truck box when a piece of slag flew up and struck his eye lid.

While removing a hydraulic hose on L-480 (#31), hose flipped out of clamps and the hose had a tag end that was sharp and struck injured in the face/nose.

While taking a production check, caring a bag of stone, injured slipped when walking down catwalk (#6 Boom) and landed on his posterior (rear end).

The injured employee was assisting in setting cables on the mill 7 ft crusher when the bulldozer operator was signaled to go "back". However, the operator thought he was signaled to go "forward", which tightened the cable, causing injured's hand to be pinched between the crusher and cable.

When reaching for oil can in back of pickup truck the injured felt strain to right side of ribs.

While in the ship garage changing a tire the injured was hooked on sharp steel.

The injured was die grinding stainless steel parts in the maintenance shop. After grinding he looked up and metal fleck' s that were on his safety glasses fell into his eye.

While jacking up (#40) 2003 Komatsu Haul Truck the injured employee stood up, hard hat flipped off, and walked into right front fender.

The injured employee was getting out of his work truck by the gas pumps when he slipped on ice, hurting his back.

The injured cut his knuckle of pointer finger on left hand while checking screens on Fine Tower, middle screen.

The injured employee was driving production haul truck #43 when he went off the side of the haul road.

While operating #41 992 K Cat Wheel Loader loading production haul trucks, the injured employee was observed by one of the truck drivers as being in a stroke like condition.

While driving production haul truck #35, the injured employee attempted to move a rock approximately 30' X 30" from the haul road.

While welding cracks in the primary crusher main frame, the injured was doing a lot of twisting and turning and felt pain in his hip with increasing pain.

The injured EE was removing decorative rocks in front of the office. While putting the rocks in the bucket of the loader, he lost his grip on the rock and the rock smashed his left index finger.

Injury was putting Hydraulic Oil in #65 (1990 Kobelco K912 Excavator LC Mark II). When injured lifted a 5-Gal. can to pour into funnel he claims injuring his back.

While loading some junk tires, the injured EE twisted wrong and hurt his back.

While installing 20 FT. of 2" suction hose in pit pond, the injured was pushing hose and extended reach. Feet being in mud, twisted back.

The injured was half way back into the surge tunnel inspecting and cataloging rust trip cable supports to replace when he tripped and fell. Nothing obvious on the floor.

Injured fell about 5 Ft. from the ladder he was working from. He was at the 92" Screen Tower laying out holes to move a beam when the feet of the ladder slipped, and he fell to the ground landing on the left foot then rolled to the right landing on his hip/tail bone.

The injured was half way back into the surge tunnel inspecting and cataloging rust trip cable supports to replace when he tripped and fell. Nothing obvious on the floor.

The injured EE was grinding steel on the cutting table, about waist high, when a piece of steel came up from the grinding wheel and went up behind from the bottom of his safety glasses, lodging in his left eye.

The injured was removing a trougher at the tail end of the weigh belt with a pry bar, his hand slipped into guard.

The injured was removing a rock from the haul road when he felt pain in his groin.

Injured advised doctor that he was lifting rock from the crusher, when he experienced a burning sensation in the groin area.

Ee was removing 6" PVC pipe sections from the top of boreholes that were stuck in the muck & used a pair of pliers to try & gain a better grip. The ee was standing directly over the pipe using both hands to apply pressure to the pliers pulling in an upward motion. The pliers came loose & the pipe came up hitting the ee in mouth & face & broke a tooth & gave him a concussion.

Injured was pulling out a drill tube with pliers when the pliers slipped off the tube striking the injured in the mouth and head.

The injured employee was removing rusted metal from back of #29 Mack truck using a 6" angle grinder and pliers when he cut his hand while reaching over the grinder with the pliers to remove the metal.

The injured was cleaning the desks at the office using an aerosal spray, when she experienced a sore throat, difficulty breathing and a pain in her lower chest.

The injured was washing out a screen to be repaired when he lost his balance with the water hose and had to jump off the 2nd step of the ladder he was standing on. He landed with one foot in a 5-gallon waste oil bucket, twisting his hips and ankle.

While exiting the 5 Foot Crusher after repairing liners, the injured hit his left elbow on a stantion (drain) pipe. Did not seek medical treatment until 4/21/2006.

The injured got a piece of metal in his right eye while operating a die grinder while repairing a shifting linkage.

THE INJURED EXPERIENCED SORENESS IN HIS BACK AFTER OPERATING THE PC400LC-7 KOMATSU AFTER DIGGING OUT LARGE ROCKS AND THE BUCKET SLIPPED OFF SEVERAL TIMES DROPPING, CAUSING JAR SENT TO THE CAB AND SEAT.

THE INJURED WAS USING A DIE GRINDER TO CUT A SOLENOID FREE FROM THE FRAME OF THE 1988 FORD MODEL CARGO 8000 FUEL TRUCK, APPLYING THE CHARGED GRINDER TO THE BOLTS OF THE MOUNTED SOLENOID, WHEN THE GRINDER "DUG IN" AND JUMPED, STRIKING THE BACK OF THE INJURED'S LEFT HAND BETWEEN THE THUMB AND INDEX FINGER RESULTING IN A 1 1/2 INCH LONG CUT.

THE INJURED BRACED HIMSELF BY HOLDING ON TO THE DOOR FRAME OF THE PC400LC-7 KOMATSU EXCAVATOR WHEN DESCENDING THE LADDER WHEN A GUST OF WIND BLEW THE DOOR SHUT PINCHING HIS RING FINGER OF LEFT HAND.

The injured came to the office on 6/21/05 and advised he was experiencing irritation in his left eye since approx. thursday june 16,2005 and not improving. injured was set up with an appointment to see eye dr,.

Fire in boat loading shuttle house.

The injured was hosing around tail end of 21 FT. feeder belt, when he accidentally "back" sprayed himself in the eyes.

The injured experienced itching, mattering, redness and watering of this left eye after cleaning in the surge tunnel.

THE INJURED EE SMASHED HIS FINGER WHILE REMOVING A COMPRESSOR FILTER WHILE WORKING ON THE DRILL.

THE INJURED EMPLOYEE WAS PICKING UP ROCK OFF THE HAUL ROAD. WHEN HE THREW A ROCK BACK OVER HIS SHOULDER HE HEARD SOMETHING SNAP.

THE INJURED WAS IN THE GARAGE HELPING CHANGE A TRACK LINE ON THE DOZER WHEN HE HAD HIS HAND RESTING FLAT ON THE GARAGE FLOOR, THE LINK FELL FROM THE LINK/TRACK PULLER, STRIKING THE BACK OF HI S RIGHT HAND.

THE INJURED EMPLOYEE HAD BEEN USING A WATER HOSE TO CLEAN AT THE #1 TUNNEL, WHEN HE WAS STARTLED BY A PUMP STARTING UP AND TURNED AND FELT SOMETHING OUT OF THE ORDINARY IN HIS BACK.

THE INJURED EE WAS WELDING IN FEEDER HOPPER OF THE COARSE TOWER. WIND WAS BLOWING SLAG FROM THE WELDS AROUND. A PIECE GOT IN HIS RIGHT EYE AGGRAVATING IT.

THE INJURED EMPLOYEE WAS ADJUSTING THE TENSION CHAIN ON A WIPER WHEN THE CLIPS ON THE BELT SNAGGED THE EDGE OF THE WIPER SWINGING THE HANDLE HITTING HIS RIGHT EYE.

THE INJURED EE WAS REPLACING LINERS IN THE 5 FT. CRUSHER AND GOT A FOEIGN PARTICLE IN HIS EYE.

THE INJURED EMPLOYEE WAS GREASING THE TOP OF THE G-3 CONVEYOR WHEN HE STEPPED ON THE TOP STEP, WHICH BROKE, CAUSING HIM TO GO THROUGH THE STEP AND INJURE HIS BACK AND KNEE.

THE INJURED EE SLIPPED ON THE ICE WHEN GETTING OUT OF HIS TRUCK, FALLING ON HIS RIGHT ARM/SHOULDER.

DRIVER STATED TO ME THAT HE FAILED TO SEE THAT HIS BOX WAS UP, AND FAILED TO SEE TRIP WIRE LIGHTS. I ASKED HIM WHAT HAPPENED AND HE SAID IT WAS A STUPID THING. I JUST WASN'T THINKING, I DIDN'T SEE IT. I ALSO ASKED HIM IF HE WAS HURT AND HE SAID, NO, I AM O.K., 1 HOUR LATER: NO, I AM ALRIGHT. NEXT DAY (MORNING) I AM O.K. THANKS.

WHEN ATTEMPTING TO FUEL #10 DOZER, THE INJURED PLACED HIS LEFT FOOT ON THE LEFT TRACK AND HIS RIGHT FOOT ON TOWING HITCH, FACING THE MACHINE, ATTEMPTING TO REMOVE THE FUEL CAP WHEN HIS FOOT SL IPPED OFF THE HITCH, FALLING TO HIS RIGHT TURNING 180 DEGREES IN MID AIR,LANDING ON HIS LEFT LEG

THE INJURED WAS ASSISTING IN REMOVING THE DRILL STARTER BAR THREADS THAT HAD BROKEN OFF IN THE SUB SPINDLE WHEN THE BAR AND HAMMER DROPPED APPROX. 2 FEET AND CAME DOWN ON THE INJURED'S FOOT.

THE INJURED FELL OVER THE CONVEYOR COVER WHICH HE WAS INSTALLING A PIECE OF RUBBER BELTING AT THE TAIL END OF THE #3 CONVEYOR, LANDING ON A CEMENT SLAB APPROX. 6 FT BELOW CAUSING COMPOUND FRAC TURE TO HIS LEFT ELBOW AND BROKEN LEFT CHEEK BONE.

INJURED TWISTED HIS RIGHT KNEE WHEN DESCENDING LOADER STEPS TO GREASE #43 CAT 980B LOADER.

EE SPINAL DISORDER DIRECTLY RELATED TO CUMULATIVE DRIVING VIBRATION AND POSTURING.

INJURED WAS WORKING ON THE FINE TOWER CHANGING SCREENS WHEN HE TURNED AND INJURED HIS BACK

WHILE REPAIRING A MILL CONVEYOR, THE INJURED'S SHIN WAS BUMPED BY A FELLOW WORKER WHILE RATCHING WITH A WRENCH ON BELT CLAMPS.

INJURED WAS WORKING AT THE 21' CRUSHER REPACKING THE OIL PUMP, WHICH ENTAILED REPEATED UP AND DOWN STAIRS.

INJURED WAS IN PATH OF REAR TRACTOR TIRE OF REVERSING 70-TON DART HAUL TRUCK.

INJURED WAS PULLING ON THE HOCKS TO UNPLUG THE HYDROCONE CRUSHER WHEN HIS ELBOW HIT THE CATWALK ABOVE, CAUSING HIS RIGHT HAND TO GO NUMB.

THE INJURED WAS CHANGING THE CUTTING EDGE ON THE MODEL 16 CAT GRADER WHEN HE HURT HIS BACK LIFTING ON THE CUTTING EDGE.

WHILE WORKING ON THE HYDRCONE SCREEN THE INJURED EMPLOYEE SLIPPED ON ROCK OVERFLOW AROUND THE 4FT CRUSHER AND FELL 5-6 FT INTO THE HAND RAIL AND TWISTED HIS LEFT KNEE.

THE INJURED WAS SERVICING OUR #31 MI L-480 LOADER, WHEN HE PULLED THE FUEL HOSE OVER THE SIDE OF THE LOADER AND GOT TANGLED UP IN THE STAIRWAY. WHIILE LEANING OUT TO PULL UP ON ROPE, EXPERIENC E BACK STRAIN.

THE INJURED WAS DRIVING DART TRUCK #21 OUT TO THE PIT TO BE LOADED WHEN HE FELL ASLEEP AND RAN OFF THE RIGHT SIDE OF THE ROAD, DOWN THRU THE DITCH AND ROLLED UP ON IT'S RIGHT SIDE.

INJURED WAS CLEARING THE AGMEAL LINE AND SLIPPED ON THE PIPE CAUSING HIS RIGHT SIDE TO BE WRENCHED.

THE INJURED WAS HOSING THE MAIN FLOOR OF THE 7 FT CRUSHER WHEN IRRITATION OF THE LEFT EYE WAS EXPERIENCED.

THEI NJURED WAS ADJUSTING A GATE ON THE SECONDARY TOWER PULLING ON A PRY BAR, WHEN HIS FEET SLIPED FROM UNDER HIM. TO PROTECT HIMSELF, MOVED HIS LEFT ARM BEHIND HIMSELF, HITTING THE EDGE OF A STANDING ANGLE IRON.

EMPLOYEE IN DART #19 DROVE TOO CLOSE TO THE RIGHT SIDE OF THE ROAD AND LOST CONTROL OF THE TRUCK. HE RAN OVER 150 FT OF BUMPER STONE AND CAME TO REST IN AN UPRIGHT POSITION IN THE DITCH. THE D RIVER WAS NOT SERIOUSLY INJURED.

WHEN WALKING OVER TO TRUCK #26 TO ADVISE THE DRIVER OF PIPE FOUND IN THE ROAD, THE INJURED EMPLOYEE STEPPED ON A ROCK CAUSING HIS FOOT TO TWIST.

WHILE FUELING DART TRUCK #20, THE HOSE FLEW APART AND DIESEL FUEL SOAKED THE SHOES OF THE INJURED.

THE INJURED WAS GETTING OUT OF THE PARKED CHAMPION GRADER. STEPPING OUT OF THE OPERATORS CAB ONTO THE DESCENDING LADDER, HIS FOOT SLIPPED ON THERUNG BECAUSE OF OIL ON HIS SHOE SOLE.HE CAUGHT HIMSELF WITH HIS RIGHT HAND ON THE DOOR HANDLE TO PREVENT FALLING & PULLED A MUSCLE IN THE NECK FROM THE STRAIN.

INJURED EMPLOYEE WAS HELPING INSTALL THE SLEEVE ON TOP OF THE CRUSHER & WAS GUIDING IT INTO PLACE SLEEVE DROPPED SUDDENLY & CAUGHT HIS GLOVE & PULLEWD HIS LITTLE FINGER ON HIS RT HAND UNDER IT THIS PINCHED THE TIP OFF OF HIS FINTER SURGERY WAS REQUIRED TO REPLACE THE SEVERED FINGER TIP

THE INJURED EMPLOYEE WAS HOOKING UP A WELDER TO A TRUCK. HE ROLLED WELDER AHEAD TO MAKE HOOKUP AND THE WELDER LURCHED AHEAD OVER A LIP IN THE CONCRETE PINCHING HIS LEFT THUMB BETWEEN THE WELD ER TONQUE AND TRUCK HITCH.

WHILE CROUCHING UNDER A DART TRUCK TO MAKE REPAIRS, THE INJURED'S RIGHT KNEE SNAPPED AND HE WAS UNABLE TO STAND ON IT.

THE INJURED WAS REMOVING A DRIVE SHAFT OF A KWDART WHEN IT SLIPPED OFF THE FLANGE STUDS,PINCHING HIS LEFT THUMB BETWEEN THE SHAFT AND HOIST PUMP HOSE.

THE INJURED EMPLOYEE WAS PULLI8NG ON A 55-GALLON BARREL OF OIL, GREASE, ETC. WHEN HIS FEET SLIPPED ON DIRT ON A CEMENT FLOOR, CAUSING THE EMPLOYEE'S INJURY TO HIS BACK.

THE INJURED EMPLOYEE WAS REMOVING A WINDHSIELD WIPER FROM THE MOTOR SHAFT ONT EH MICHIGAN DOZE.R WHEN THE WIPER ARM BROKE FREE, HE WAS IN A SQUATTING POSITION, AND LOST BALANCE CAUSING HIM TO FALL APPROX. 9 FT DOWN TO THEBLADE. CLIMBED BACK ONTO THE MACHINE, FINISHED THE JOB, CLIMBED AROUND TO GET DOWN BY THE LADDER. WHEN HE STEPPED ONTO THE LADDER WITH HIS RIGHT LEG, HIS RIGHT

REPLACING RETURN ROLLERS ON 2 CONVEYOR. AFTER HOOKING HIS SIDE, THE INJURED EMPLOYEE WENT UNDER THE CONVEYOR, STANDING ON THE CROSS MEMBERS LIFTED THE BELT SO HIS FELLOW WORKER COULD HOOK THE END OF THE ROLLER IN THE BRACKET.

EMPLOYEE WAS IN3THE GARAGE SHOVELING OUT THE EAST FLOOR GUTTER, AND STRAINED HIS BACK MUSCLES.

THE INJURED WAS3INSTLLING CABEL IN ROPE SOCKET WHEN THE END WHIPPED OUT STRIKING HIM ON THE HEAD OVER HIS RIGHT EYE CUTTING IT OPEN.

WHILE RUNNING T2E HOUGH, THE INJ EMP, WAS DIGGING ROCK ON T HE QUARRY FLOOR, HE RAN THE BUCKET INTO THE BANK HITTING A BIG ROCK, TWISTINGK HIS BACK

MAN WAS GETTING3OUT OF MAINTENANCE TRUCK SWUNG LEGS OUT & HIS BACK WENT OUT.

THE INJURED EMP5OYEE WAS ON THE TO LEDGE OF THE QUARRY MARKING OUT A SHOT. BENT DOWN AND LIFTED A STONE TO MARK DRILL HOLE WHEN HE INJURED HIS BACK.

GETTING A TIRE 9EADY FOR DART TRUCK 19 THE TIRE WAS LEANING AGAINST THE RACK BUT STANDING TOO STRAIGHT UP IT STARTED TO ROLL AND HE TRIED TO HOLD IT UP HE COULDNT HOLD IT AND IT FELL OVER ON HIM STRIKING H3M ON THE KNEES

THE INJURED WAS3WORKING ON THE SECONDARY TOWER PUTTING CLIPS IN THE G-1 CONVEYOR BELT.HE WAS WALKING ON THE CATWALK WHEN HE SLIPPED ON A STONE,ROLLING HIS LEFT ANKLE.

THE PERATOR SET3AND ADJUSTED THE DRILL HE WAS USING (NO.3)AFTER GETTING THE DRILL ADJUSTED,THE COUPLING BROKE,WHEN HE WENT TO SHUT THE DRILL DOWN A PIECE OF THE COUPLING LODGED H.IN HIS ARM.

EMPLOYEE WAS DU4PING A LOAD AT THE PRIMARY CRUSHER, PUSHED DOWN ON THE DUMP LEVER TO DISENGAGE POWER TAKE OFF, IT STUCK, SO HE PUSHED DOWN REAL HARD, HURTING HIS SHOULDER.

EMP HAD BEEN IN3DRIVERS SHACK WHEN THE BLAST WENT OFF HE LEFT THE SHACK. AS HE STEPPED OUT A ROCK STRUCK THE GROUND, RICOCHETED OFF THE SIDE OF THE SHACK, STRUCK MAN IN THE LEG CAUSING ACCIDEN

EMPLOYEE WAS ST5NDING ON TRACK OF LIMA CRANE(INSTALLING RAIN CAP ON EXHAUST PIPE) ATTEMPTING TO CLIMB TO UPPER LEVEL.WHILE DOING SO HIS FOOT SLIPPED CAUSING INJURY TO LEFT KNEE.

THE INJURED EMP5OYEE WAS WORKING ON THE DOOR OF A PICKUP TRUCK AND WHEN HE BENT OVER TO PICK UP A TOOL COULD NOT STRIGHTEN UP.

INJURED WAS ATT9MPTING TO POSITION A 55 GAL OIL DRUM ON A HORIZONALPEDESTAL FOR DISPENSING HIS LEFT HAND WAS PINCHED BETWEEN THE DRUM AND THE PEDESTAL
