EE was hauling tailing pond waste in a Volvo A60. When the EE attempted to dump the material about 30 tons remained frozen in the box and when putting the box back down it slammed hard, shaking the cab and operator. EE did not seek medical attention at the time of the incident. Work restrictions issued on 2/5 following ongoing discomfort and medical treatment.

ee was assisting with a belt pull. When pushing on the belt, felt a pop in EE's knee. was evaluated and was put off work.
ee was climbing up stairs; reached out to grab the handrail and felt pain in middle back area.
after drawing out feeder chute and running it dry, ee locked out belt and feeder and crawled up into chute to investigate a potential obstruction. After poking around with a pry bar, material from the overhead stockpile sluffed off and engulfed ee in the chute. EE was extracted and is recovering at home (lacerations and contusions)
employee was using a long pry bar to hit the side of the chute to help clear the built up stone. EE was twisting while struck the chute and felt a sharp pain in EE's lower back.
ee was moving a diesel pump in the quarry. ee picked up the suction hose and felt a strain in their back.

EE was walking across the parking lot when EE slipped on the ice and fell. EE's face made contact with the ground, causing EE's safety glasses to push into EE's forehead resulting in a small laceration above EE's eye that required 4 stitches. No restrictions issued- cleared to RTW.
EE was using a torch to cut the cutting edge on a loader, a large piece of it fell and landed on EE's left foot resulting in a fracture.
EE picked up a large stone to remove it from the roadway. As EE picked it up, it broke into pieces. A sharp edge of the stone cut the ee's finger. Laceration requires stitches. No further restrictions or treatment administered.
EE lost control of haul truck & veered off haul road. Operator sustained minor bruising from seatbelt. Cause of incident under investigation - weather conditions unfavorable at the time.
On 2-1-24 EE reported to EE's foreman that on the previous work day1-31-24, EE's buttocks were burned to the point of blisters. EE said these were caused by the seat warmer. When asked why EE did not turn off the seat warmer, EE replied, "I did not think it was that hot"

Injured was returning to the quarry with unloaded Cat tractor/trailer haul truck. Injured was momentarily distracted by an unidentified object on the left side of the haul road which caused EE to drift toward the rock berm on the right side. The right rear trailer tires struck a rock which caused EE to lose control and exit the road.
A team of 4 employees was putting a pulley on #1 scalper. When ratcheting on a chain fall, the pulley slid/moved. Injured was not directly engaged with the pulley, but jumped backward when the pulley shifted and jarred EE's back.

Injured was using a bar to clear a chute at the 21 ft. crusher when EE felt pain in the wrist. Injured did not report the incident until 08/31 when EE requested medical treatment.

Employee states that as EE was walking from EE's vehicle to the office to clock in, EE tripped over a piece of wood sticking out of the ground from a sign post that had previously broken. As EE fell, EE states that EE landed on EE's left arm
Employee was accessing a haul truck by means of a cable type 3 step ladder when EE's foot slipped off the step and EE took the brunt of EE's weight with EE's left arm resulting with a strain to upper left arm.
Miner was performing clean-up activities using a water hose in a limited headroom area. As the miner was washing, EE hit head which caused EE to fall to the deck and breaking left leg, requiring surgery.

Employee suffered muscle strain and severe bruising after steering shaft broke on 768C haul truck. Broken shaft ripped supplemental steering harness out as well, leaving driver only able to hold on as truck ran off haul road into drainage ditch. Truck stayed on wheels at all times.

While removing a hydraulic hose on L-480 (#31), hose flipped out of clamps and the hose had a tag end that was sharp and struck injured in the face/nose.
While taking a production check, caring a bag of stone, injured slipped when walking down catwalk (#6 Boom) and landed on his posterior (rear end).

The injured employee was assisting in setting cables on the mill 7 ft crusher when the bulldozer operator was signaled to go "back". However, the operator thought he was signaled to go "forward", which tightened the cable, causing injured's hand to be pinched between the crusher and cable.
When reaching for oil can in back of pickup truck the injured felt strain to right side of ribs.
While in the ship garage changing a tire the injured was hooked on sharp steel.
The injured was die grinding stainless steel parts in the maintenance shop. After grinding he looked up and metal fleck' s that were on his safety glasses fell into his eye.
While jacking up (#40) 2003 Komatsu Haul Truck the injured employee stood up, hard hat flipped off, and walked into right front fender.
The injured employee was getting out of his work truck by the gas pumps when he slipped on ice, hurting his back.

While welding cracks in the primary crusher main frame, the injured was doing a lot of twisting and turning and felt pain in his hip with increasing pain.
The injured EE was removing decorative rocks in front of the office. While putting the rocks in the bucket of the loader, he lost his grip on the rock and the rock smashed his left index finger.
Injury was putting Hydraulic Oil in #65 (1990 Kobelco K912 Excavator LC Mark II). When injured lifted a 5-Gal. can to pour into funnel he claims injuring his back.
While loading some junk tires, the injured EE twisted wrong and hurt his back.
While installing 20 FT. of 2" suction hose in pit pond, the injured was pushing hose and extended reach. Feet being in mud, twisted back.
The injured was half way back into the surge tunnel inspecting and cataloging rust trip cable supports to replace when he tripped and fell. Nothing obvious on the floor.
Injured fell about 5 Ft. from the ladder he was working from. He was at the 92" Screen Tower laying out holes to move a beam when the feet of the ladder slipped, and he fell to the ground landing on the left foot then rolled to the right landing on his hip/tail bone.
The injured was half way back into the surge tunnel inspecting and cataloging rust trip cable supports to replace when he tripped and fell. Nothing obvious on the floor.

Ee was removing 6" PVC pipe sections from the top of boreholes that were stuck in the muck & used a pair of pliers to try & gain a better grip. The ee was standing directly over the pipe using both hands to apply pressure to the pliers pulling in an upward motion. The pliers came loose & the pipe came up hitting the ee in mouth & face & broke a tooth & gave him a concussion.
Injured was pulling out a drill tube with pliers when the pliers slipped off the tube striking the injured in the mouth and head.
The injured employee was removing rusted metal from back of #29 Mack truck using a 6" angle grinder and pliers when he cut his hand while reaching over the grinder with the pliers to remove the metal.
The injured was cleaning the desks at the office using an aerosal spray, when she experienced a sore throat, difficulty breathing and a pain in her lower chest.

The injured was washing out a screen to be repaired when he lost his balance with the water hose and had to jump off the 2nd step of the ladder he was standing on. He landed with one foot in a 5-gallon waste oil bucket, twisting his hips and ankle.
While exiting the 5 Foot Crusher after repairing liners, the injured hit his left elbow on a stantion (drain) pipe. Did not seek medical treatment until 4/21/2006.
The injured got a piece of metal in his right eye while operating a die grinder while repairing a shifting linkage.

THE INJURED WAS USING A DIE GRINDER TO CUT A SOLENOID FREE FROM THE FRAME OF THE 1988 FORD MODEL CARGO 8000 FUEL TRUCK, APPLYING THE CHARGED GRINDER TO THE BOLTS OF THE MOUNTED SOLENOID, WHEN THE GRINDER "DUG IN" AND JUMPED, STRIKING THE BACK OF THE INJURED'S LEFT HAND BETWEEN THE THUMB AND INDEX FINGER RESULTING IN A 1 1/2 INCH LONG CUT.
THE INJURED BRACED HIMSELF BY HOLDING ON TO THE DOOR FRAME OF THE PC400LC-7 KOMATSU EXCAVATOR WHEN DESCENDING THE LADDER WHEN A GUST OF WIND BLEW THE DOOR SHUT PINCHING HIS RING FINGER OF LEFT HAND.
The injured came to the office on 6/21/05 and advised he was experiencing irritation in his left eye since approx. thursday june 16,2005 and not improving. injured was set up with an appointment to see eye dr,.
