Employee was cleaning the plant area, rolled a 2" x ~15' suction hose, and placed on the floor. At the time the hose was placed on the floor he apparently twisted his back. An 11/9/15 physician's report indicates that the employee's diagnosis was "Lumbosacral strain / R Sciatic neuralgia", and states that the employee "is totally incapacitated at this time".

As the employee attempted to remove cattle from the mill site, a cow "charged" the employee. The employee jumped off an embankment to avoid the cow and injured his heel as he landed on the roadway. Accident did not involve compliance issues, mine equipment or mining system, or miner related skills training or attitude. Required PPE was utilized at the time of injury.

Worker was working by air compressor fixing jig screens. The extra jig screens were resting against compressor. The compressor fell over, breaking one of the fittings. It caused a blast of air, the worker ended up on the floor-five feet from original position, and a cloud of dust occurred. the worker got metal fragments in his eye. He had his safety glasses on.

Employee was shoveling mud (barite) up off the floor into the shaker (dewatering screen), which is about 5 foot high and his knee gave out on him. This is a repetition job to keep his area clear of hazards and clean as needed.

Upset employee, wouldn't fill out paper work, tried to bully supervisor, supervisor got in pickup to leave, employee stuck his foot under pickup tire. Tire ran over foot, nothing broken in foot - bruised (hospital x-ray).
