On 8/14/2013 worked full day had a little back soreness didn't think anything of it. On the morning of 8/15/2013 back hurt severely, worked the full day running the loader only (no lifting). I think the cause was from frequent bending and lifting on 8/14 and it just caught up to me.

Stone rolled onto left hand smashing it. Left index finger broken. Surgery was done to repair finger. Employee be out of work for 6 to 8 weeks.
