EMPLOYEE WAS PULLING ON A 3/4 RATCHET WRENCH. THE DIRECTION INDICATOR WAS NOT ALL THE WAY IN THE TIGHTENING DIRECTION. THE WRENCH SKIPPED A TOOTH AND THE EMPLOYEE STRAINED HIS BACK. **DID NOT SEEK MEDICAL ATTENTION UNTIL JULY. NO PRESCRIPTIONS OR PHYSICAL THERAPY WERE GIVEN UNTIL LATER AT WHICH TIME IT BACAME REPORTABLE.

AFTER WE FINISHED BURNING THE EE BEGAN REMOVING THE SMALL PIECES OF THE DRILL BIT THAT WAS LEFT. HE HAD THICK WELDING GLOVES AND WAS THROWING HANDFULLS ATA TIME OUT AS HE WAS THROWING THE TOP OF HIS LT.HAND STRUCK THE CHUTE ABOVE CUTTING HIS LT. HAND

EE WAS WEARING CUTTING GLASSES WHICH LEFT OPEN AREAS FOR FOREIGN MATERIALS TO GET BEHIND THEM. THE EE WAS USING A CUTTING TORCH CUTTING A SHANK OFF OF A BUCKET. THE SLAG POPPED AND A SMALL PIE CVE OF SLAG GOT BEHIND HIS CUTTING GLASSES AND GOT TO HIS EYE. WE TOOK THE EE TO THE ER AND THE DR. REMOVED IT AND SENT HIM BACK TO WORK.

EMPLOYEE WENT TO PIT WELD ON HYD HAMMER ON EX 400 TRACKMACHING. HE WENT TO THE OPERATOR AND ASKED HIM TO MOVE MACHINE AWAY FROM WORK AREA. THE EMPLOYEE WALKED BACK TO THE WELDING TRUCK WHICH W AS 50FT AWAY. WHEN THE OPERATOR STARTED TO WALK THE MACHING A SLIVER OF METAL SHOT FROM THE TRACK AREA AND HIT EMPLOYEE IN THE LEFT FOREARM. EMPLOYEE HAD TO HAVE SURGERY AT HOSPITAL.

EE WAS DIGGING LEFT CENTER OF SHOT IN FLOOR. HE WAS ATTEMPTING TO MOVE A BOULDER ABOUT 4' UP INTO THE SHOT. WHEN IT MOVED, A SMALL ROCK CAME FROM BEHIND IT AND WENT THRU THE WINDSHIELD. AS THE ROCK CAME THRU HE PUT HIS HANDS UP AND DIRECTED IT TOWARDS THE FLOOR OF THE LOADER. THE ROCK STRUCK HIM ON HIS SHINS AND CUT HIS RIGHT SHIN. IT TOOK 9 STITCHES TO SEW IT UP.
