WHILE DOING A PRESHIFT INPSECTION ON THE ALLIS CHALMERS SCREEN, EE SLIPPED ON A ROCK ON THE WALKWAY CAUSING HIM TO FALL INTO THE SCREEN HITTING HIS LEFT SIDE FRACTURING A RIB AND BRUISES TO HI S SIDE.

WHILE EE WAS PICKING UP A BALE OF STRAW, HE STEPPED INTO A HOLE BETWEEN THE BALES CAUSING HIM TO LOSE HIS BALANCE. HE FELL OFF THE FLATBED TRUCK APPROX. 4 FT TO THE GROUND LANDING ON HIS LEFT WRIST CAUSING THE FRACTURE.
