MR FOERMAN WAS USING A TOP SECTION OF AN EXTENSION LADDER (NOR RUNG WHERE THE HOOKS HOLD THE TOP SECTION OVER THE LOWER SECTION) TO GET UP AND DOWN FROM THE PREPARATION PLANT HE WAS WORKING ON . AS HE DESCENDED THE LADDER HE STEPPED THROUGH THE HOOK AREA AND FELL ABOUT 2\ TO THE GROUND FALLING ON HIS RIGHT SHOULDER AND ELBOW.
