ERP Iron Ore-Bovey Plant: 91 MSHA citations Metal/Non-Metal

A contracted employee was climbing out of a semi-truck when he missed the last step falling to the ground. The contracted employee had his hands full and did not maintain three points of contact while de-boarding.

A miner was threading a 2" in diameter pipe. The rotating pipe became entangled with the miners sweatshirt sleeve, throwing the miner to the ground. The miner landing on his left shoulder. Possible left shoulder separation/sprain (waiting on MRI).

Injured was taking out the garbage. When he pushed down on the garbage can liner to compact the contents a tin soup can lid penetrated the bag cutting his hand.

Employee was holding on to the drive end of a pump while another employee was turning the pump impeller into the shaft. The employee's fingers got pinched when the impeller seated down on the shaft.

Employee was driving a Loader to the hopper when the right corner of the loader bucket caught the lip of the light post. Employee bumped his head on the load rite scale and cut his right eyebrow area.

Injured was grinding the broken weld off the face plate of the froth thickener rake when the grinder cocked and kicked back and hit the injured guide hand.

Employee was cutting a plastic sign with a utility knife. Something slipped causing the knife to cut the employees left index finger.

Injured exited pick-up truck he was operating twisting left knee in the process. Note: Employee stated, on 4-13-15 that the injury was sustained on property. This was the first notification of the injury, provided to Magnetation.

Employee was lifting the dogbone while walking backwards and his knee locked up.

Employee looked to the left while loading a truck, felt a pop and could not look down or to the left without pain.

2 employees were removing a guard. Turned left to place guard on floor grating and felt a pop in his mid back. Body mechanics was the contributing factor of the injury. The guard was small and light weight.

Employee reported the repetitive motion of flushing Rev #1, caused pain in his right shoulder.

Walking across Plant floor and rolled right ankle. Witness claims there wasn't a slip, trip, or fall. The incident was reported on 7/27/14 to the safety department. Diagnosed with a sprained ankle/foot. Employee received a x ray on 8/28/14 due to residual pain, and was told that a fracture may have occurred. Waiting for recommended CT scan and final doctor notes.

Employee putting side cutters (approximately 25-30 lbs.) on backhoe bucket, felt pain to lower back.

Injured was removing a bad magnet by hand from rev 1 lower deck with help from a co worker. They lost control of the magnet. Injured attempted to try and regain control causing what was believed to be a strained shoulder. Injured didn't feel medical attention was necessary at the time. Note: 4-8-15 notification was given that the injured would undergo surgery for a torn labrum.

Step off Cat 740B Haul Truck to clean back up camera and slipped on ground, twisted left knee, climbed back into cab - reported to shifter at end of shift - June 2, 2014 7:00AM

Driving pin out of drive support sprocket on brush drive shaft of street sweeper; hitting punch with sledge hammer, punch flew back at employee after striking with sledge hitting him in the upper lip, splitting lip and breaking front upper two teeth.

EE bent down to attach Chicago coupling to discharge end of pipe - felt tightening and pain to back of right knee.

Employee fainted while watching training videos

Operate haulage truck, claimed driving equipment caused inflammation to scrotum. Employee was off 6 days - scheduled work days were March 3-6, March 11-14. Employee returned to full duty on March 18th. Question of work related injury. Validity of claim is being investigated

Standing on ladder, EE was tightening bolts when his right calf erupted in pain.

Smashed "R" middle finger between 300 lbs. magnet and other magnet

Stepping out of man basket-felt right knee "pop"

Individual was performing a test in the Lab on the Slon bench tester when the magnet was turned on and the fill tank came down on his right pointer finger. The finger was smashed and it caused an open wound on the inside of the digit. He received 7 stitches at the local hospital.

Two Hammerlund Construction 773 haul trucks were hauling fill for a road build on an internal impoundment. The bulldozer operator noticed the bank giving way and called on radio trucks to get out, one truck slid approximately 20 feet on its wheels and rested while the full truck flipped on its top and slid 20 feet. There were no injuries due to the incident.

While moving a part to the Rev the edge of the hammer head was sharp causing a laceration to his right thumb, leather gloves were worn and 5 stitches were required.

While tightening a nut on a bolt, the wrench slipped causing a laceration on his knuckle which required 4 stitches

While climbing down the ladder of the excavator, his back tightened up causing leg pain and numbness

Employee was helping remove ice from an 8" poly pipe that was elevated 3' off of floor when the ice moved the pipe hit him in the right knee causing a knee sprain

Employee was walking from the equipment hill to plant and while doing so stepped in a small hole twisting his ankle causing for a sprained ankle.
