Employee was performing maintenance work on an electrical motor that drives the screen deck. After losing balance when the nut/bolt started to loosen, they caught himself from falling with their left arm.

Employee was working with hand wrench and impact driver to loosen bolt. Lost grip on wrench open end whipped around caught EE on the side of forearm. Onsite medical treatment was provided, however, on 7/10/24 the employee notified us that EE thought the laceration was infected. We took EE to the Dr. and it was, so they gave EE some antibiotics.

Patching belt using a personal knife. cutting patch knife slipped and they cut their knee.

We were using a propane torch to thaw frozen material underneath the cone crusher. The torch was left under the crusher with a small flame overnight and it ignited the conveyor belt causing a fire burning the belt and tires on the chassis.

During a maintenance shut down, employee was stepping onto a belt in order to access the dust mitigation system. As he stepped onto the working surface he did not notice that it was slippery. He slipped and fell to a sitting position and contacted a 12" vertical metal "skirt board" with his back and left side. Upon Dr. exam and x-ray, was found to have one fractured rib.

Sunny winter weather, using hand-held grinding wheel to take surface rust off of 2" steel tubing, wheel caught on edge and kicked back onto left hand causing a cut requiring six stitches. Able to go back to work with no restrictions.

Maintenance worker replacing a guard on screen deck and got hand between guard and spring mount, guard shifted, pinched hand causing small cut on outside of left palm area. 5 stitches, no lost time.

A 30 HP MOTOR WAS BEING LIFTED BY JACK ON THE FREE FLATING END SO NEW BELTS COULD BE INSTALLED. THE JACK HAD JUST REACHED THE HEIGHT NEEDED AND AS IT WAS ABOUT TO BE BLOCKED IT SLIPPED OFF PIN CHING 3 FINGERS ON THE LEFT HAND OF THE MECHANIC
