Collier Materials Inc · Fall from machine

Employee said they fell from the loader while mounting the loader. The employee said they fell onto their belly. The employee showed me a three scratch marks, two on EE's belly and one on EE's right arm. Employee complained of right leg hurting and not being able to straighten leg. I suggested to employee that we apply ice to their knee, employee refused, drove home.
