The drive pulleys on the conveyor had become built up with frozen material. The conveyor was started to make sure all of the frozen material had been removed. A large pry bar was left on the conveyor belt when it was started. EE saw it and reached in to grab it when it was grabbed by the drive pulley and smashed EE's hand against the side of the conveyor frame.
