employee was tested for hearing and displayed a shift of 30 dec. in his right ear and a shift of 10 dec. in his left ear. Employee had a history of ear problems before starting employment here. He has been scheduled for a full hearing evaluation on 4/18/08.

Unloading guillotine cutter and stone fell and pinched finger between two stones.

Employee was operating stone splitting machine when he placed his hand under a piece of stone and lowered the machine head onto stone, pinching his fingers.
Employee was discarding scrap stone into a wastebin and pinched his finger between the steel tub and a piece of stone. He crushed his knuckle, had a compound fracture and severed the tendon in the little finger of his left hand. The little finger was amputated to the second joint.

EE WAS HELPING UNLOAD SAW WHEN A BLOCK OF STONE FELL BACK ON TO EE FOOT.
WHILE EE WAS OPERATING COBBLE SPLITTER, A STONE ROLLED. WHILE EE WAS TRYING TO STOP THE STONE, THE STONE PINNED EE'S HAND AGAINST THE TABLE.
WORKER STEPPED ON WET, SLIPPERY STONE AND FELL, INJURING HIS RIGHT KNEE.
