EMPLOYEE WAS MA5ING A ROAD ALONG SIDE A ROCK DUMP WITH A D-10 DOZER. IT WAS RAINING, WINDOWS OF D-10 WERE FOGGED, SPOIL WAS SLIDDING IN BEHIND THE D-10 WHICH HE WAS UNABLE TO SEE. A BOULDER FELL, THE D-10 2IT THE BOULDER AND HE WAS THROWN AGAINST WINDOW OF D-10.

I.NAME WAS HELP3NG REPAIR THE FINAL DRIVE ON LOADER. HE WAS ON T HE ER SIDE OF THE LOADER MOVING FROM TH BUCKET TO THE TIRE. I.NAME WAS UNAWARE THAT THE TIRE WAS RAISED OFF THE GROUND & TURNA BLE. WHEN HE JU4PED ONTO THE TIRE THE TIRE TURNED & I.NAME FELL ONTO THE BOOM OF THE LOADER.
