A CAT 773 Haul Truck was passing through the North pit gate, at the same time a CAT 773 was traveling the same route in the opposite direction. As the two vehicles passed, the catwalk on the North bound truck struck the rear fender of the South bound truck.

Two employees were attempting to drive a pin from the bucket of a Samsung SE280 trackhoe. The employee swinging the twenty pound sledgehammer missed the pin and lost his hold on the hammer. The hammer flew approx. 8 feet, striking another employee in the leg. This resulted in a fracture.
Employee was attempting to put a block under a dozer track when the track moved and caught his right thumb between the block and track. The thumb was fractured and had tendon damage.

The employee was helping in the replacement of a adjusting wedge on a pioneer jaw crusher. The employee was attempting to pry the wedge into position when it came unhooked from the come-a-long. The wedge fell striking the employee in the head. The employee suffered a cut over his eye and one on his finger.

EE STATED THAT AS HE STEPPED ONTO THE WALKWAY HIS RT FOOT GAVE AWAY UNDER HI M, CAUSING HIM TO FALL ONTO THE WALKWAY. THE WALKWAY WAS DRY AND FREE OF ANY BUILDUP OF MATERIAL. EE WAS TAKEN TO DR FOR X-RAY, WHICH SHOWED TWO SMALL FRACTURES OF THE FIFTH BONE OF THE RT FO

EMPLOYEE WAS IN THE PROCESS OF REPAIRING A CONVEYOR BELT WHEN THE KNIFE HE WAS USING SLIPPED, CAUSING A LACAERATION ON HIS LEFT HAND. EMPLOYEE WAS TAKEN TO FLEMING CO. HOSP. WHERE THE INJURY W AS TREATED & STITCHES APPLIED. EMPLOYEE RETURNED TO WORK UPON LEAVING THE HOSPITAL.

HE WA8 ADMITTED TO HOSPITAL ON 2-15-87. HIS DEPARTMENT WAS NOT WORKING THE WEEK OF 2-16-87-2-20-87 DUE TO WEATHER CONDITIONS AND NOT SURE WHEN WILL BE ABLE TO REUTURN TO WORK BECAUS
