A CAT 773 Haul Truck was passing through the North pit gate, at the same time a CAT 773 was traveling the same route in the opposite direction. As the two vehicles passed, the catwalk on the North bound truck struck the rear fender of the South bound truck.

Using acetylene torch to remove pin, burnt into oil cavity of the pin causing the hot oil to flash and come back on burning face

A body was found in the West pit area on 11/20/2012 at approximately 14:00 hours at the Flemingsburg Quarry. Investigation is ongoing.

Two employees were attempting to drive a pin from the bucket of a Samsung SE280 trackhoe. The employee swinging the twenty pound sledgehammer missed the pin and lost his hold on the hammer. The hammer flew approx. 8 feet, striking another employee in the leg. This resulted in a fracture.

Employee was attempting to put a block under a dozer track when the track moved and caught his right thumb between the block and track. The thumb was fractured and had tendon damage.

The employee was helping in the replacement of a adjusting wedge on a pioneer jaw crusher. The employee was attempting to pry the wedge into position when it came unhooked from the come-a-long. The wedge fell striking the employee in the head. The employee suffered a cut over his eye and one on his finger.

EE WAS PUTTING HYDRAULIC OIL IN SHOVEL WHEN HE TWISTED HIS BACK.

EE WAS STOCKPILING AT TIME OF INJURY. AFTER PULLING UNDER THE 57 BIN, EE EXITED HAUL UNIT ONTO THE LOADOUT WALKWAY. EE STATED THAT AS HE STEPPED ONTO THE WALKWAY HIS RT FOOT GAVE AWAY UNDER HI M, CAUSING HIM TO FALL ONTO THE WALKWAY. THE WALKWAY WAS DRY AND FREE OF ANY BUILDUP OF MATERIAL. EE WAS TAKEN TO DR FOR X-RAY, WHICH SHOWED TWO SMALL FRACTURES OF THE FIFTH BONE OF THE RT FO

EE WAS BACKING HAUL UNIT IN STOCK PILE AREA WHEN A CUSTOMER ENTERED THE EE'S BLIND SPOT DIRECTLY BEHIND HAUL UNIT. CUSTOMER WAS DRIVING HIS PERSONAL MINI-VAN, PULLING A SMALL TRAILER. EE CONTI NUED BACKING UNTIL COLLISION OCCURED.

EMPLOYEE WAS IN THE PROCESS OF REPAIRING A CONVEYOR BELT WHEN THE KNIFE HE WAS USING SLIPPED, CAUSING A LACAERATION ON HIS LEFT HAND. EMPLOYEE WAS TAKEN TO FLEMING CO. HOSP. WHERE THE INJURY W AS TREATED & STITCHES APPLIED. EMPLOYEE RETURNED TO WORK UPON LEAVING THE HOSPITAL.

EMPLOYEE WAS USING A BAR REPLACING BELT LACING. THE BAR WAS CAUGHT INTO TAIL PULLEY, PULLING BAR OUT OF EMPLOYEE'S HAND. THE BAR CUT BOTH HANDS OF EMPLOYEE.

EMPLOYEE WAS REPAIRING TRACK ON A TRACKDRILL. AS EMPLOYEE WAS HELPING TIGHTEN TRACK, HE BACKED UP INTO PICKUP THAT WAS NEARBY.

EMPLOYEE WAS ENGAGING 440 SWITCH BOX TO AIR COMPRESSOR IN SHOP WHEN STARTING EMPLOYEE NOTICED BOX HUMMING & STARTED TO DISENGAGE WHEN BOX EXPLOADED BURNING RT HAND

EMP. WAS DRILING WITH AIR TRACK ON LEDGE AND ROCK FELW AND STRUCK EMP. IN LEFT EYE. EMP. WORKED REST OF DAY AND FELT DISCOMFORT THAT NIGHT,, RETURNED TO WORK NEXT DASY AND TEN WENT TO DOCTOR.

EMPL DRILLING F2R SHOT,LOADED SHOT WITH AMONIA NITRATE AND DYNAMITE,COMPLAINED OF NUMBNESS IN FACE AND ARM BEFORE NOON AND WENT TO DR PUT IN HOSPITAL FOR OBSERVATION.

EMPLOYEE WAS OP9RATING D8 CAT DOZER TO STRIP, THE HYDRAULIC HOSE BURST AND OIL BLEW ON HOT MANIFOLD, CAUSING A FIRE, WHICH BLEW BACK ON THE EMPLOYEE, CAUSING 1ST AND 2ND DEGREE BURNS ON HIS FA CE, HEAD. HE WA8 ADMITTED TO HOSPITAL ON 2-15-87. HIS DEPARTMENT WAS NOT WORKING THE WEEK OF 2-16-87-2-20-87 DUE TO WEATHER CONDITIONS AND NOT SURE WHEN WILL BE ABLE TO REUTURN TO WORK BECAUS
