Employee was cleaning the VSI deck and was using a fork truck mounted man basket. Employee went to enter the man basket through the walkway and slipped on the man basket floor. The floor was wet due to rainy conditions. Employee was seen by medical clinic and was diagnosed with a left knee sprain. employee was placed on restrictions resulting in a loss time accident.

Employee was assisting with patches being hoisted to fix duct work. Employee was on the 5th step of a 12 foot ladder assisting. While employee was on the ladder the ladder twisted and the employee fell to the ground landing on left wrist. Employee was taken to the emergency room and was transferred to larger hospital for surgery. Employee suffered a broken radius and Ulna.

Employee was gathering sand samples in a 5 gallon bucket. Bucket weight approx. 10 pounds. Employee removed the bucket from the bed of a pickup truck over the bed rail. Employee then noticed a pain in back that progressed through the day. Employee reported incident next day to supervisor and was taken to clinic. Placed on restriction until clearance.

Employee was using a bobcat to clear material around the dry plant. Employee was unable to close the door to the equipment and tried to force the door closed. Employee broke the glass which resulted in a laceration to right forearm.

A maintenance worker was rebuilding a 12X14 pump and case. While working on the pump, it slid off the pallet and landed on top of foot. The employee was taken to the emergency room by a co-worker. X-rays showed no fractures found in the foot. The doctor did not prescribe any medications at this time. The employee was taken off work after the follow-up appointment.

(FDF) Francis Drilling Fluids driver raised the safe rack to close EE's lids due to the way EE was lined up. EE got back on EE's truck to close EE's lids and fell to the ground.

On 12/18/2015 an associate stepped out of the Loader he slipped on uneven ground and grabbed the handle to stop from falling. We took him to the clinic where he was released without restrictions to come back to work. On 1/4/2016 an MRI was performed resulting in a torn bicep muscle. He was released without restrictions but we have restricted his tasks.

Employee ran over rock with front end loader, causing minor back pain. He was taken to Heart of Texas Hospital in Brady, Texas, and was put on restricted duty until he has follow up on 11/6/2015. We have accommodated his restrictions and is currently working under those restrictions.

Associate was breaking rocks with a loader bucket. While doing so, he started feeling light headed and nauseated. He was taken to emergency room and was diagnosed with a migraine headache, and given a prescription. He has not missed any work because of this, but has restrictions until 10/12/15.

Employee was using a pocket knife to cut a zip tie. While doing so, the knife slipped and punctured his hand. This resulted in stitches from the local emergency room. He returned to work, but was instructed to follow up with a hand surgeon for possible nerve issues. At this time, he is on restricted duty, but there has not been any lost time.

Associate was washing hot sand from under the dryer. While doing so the splice on the hose he was using broke, causing water to hit the hot sand spraying sand and steam on the associate. The associate suffered first degree burns to his left arm, neck, and face.

Associate was backing up a 770 CAT haul truck to stockpile. While backing up she was run into by a 980 CAT loader. She complained of pain and was taken to company doctor for evaluation. She was released back to work without restrictions, but refused and seeked further evaluation on her own. She was released back to work with restrictions. Case is currently under investigation

On July 6, 2012 an employee scheduled to work the 2nd shift had a bad reaction to his prescription medication. The employee became dizzy causing him to fall and strike his head against a step. The employee was treated at the local hospital where he received stitches to his forehead. The employee was released from the hospital without restrictions.

Miner was welding. He did not get his hood down quick enough and got weld flash in his eyes. It did not bother him until some time later. He went to the emergency room and the doctor gave him eye drops with gentamicin sulfate. The ER doctor told him to see an eye doctor. At 9 am he saw the eye doctor who stated that there was nothing wrong with him.

EE was leaving the dryer area and got sand in his gloves. The sand was hot and he dumped it out but ended up with burns on both hands. He was taken to the emergency room. No treatment was given but the doctor gave him a prescription to prevent infection and a pain killer. EE returned to work and has not missed any work.

Employee had a heart attack while at work. Crew leader discovered him slumped over his work station and called for help. CPR was administered by plant employees until the Brady Emergency Medical Technicians arrived to continue. He was pronounced dead at the Brady Hospital Emergency Room by Dr. around 11:30 pm.

Truck driver for an independent contractor left plant at 8:11PM. No issues observed;he drove out of sight. His wife called around 12AM looking for him. Inquiry was made to his location; he was found in vehicle parked on an access road to mine. He was found non-responsive. 911 was called & CPR administered. He was declared dead at hospital from an apparent heart attack.

EMPLOYEE WAS REMOVING A BOLT FROM A CUTTING EDGE ON A SKID STEER LOADER. THE BOLT HEAD WAS VERY SHARP AND CUT THROUGH HIS GLOVE. FOUR STITCHES WERE REQUIRED. HE RETURNED TO WORK.

SLIPPED ON WET CONCRETE, DID NOT BOTHER HIM WHILE AT WORK. HE WENT ON VACATION, RETURNED TO WORK, IT WAS BOTHERING HIM SLIGHTLY BUT HE IGNORED IT. FINALLY ON 9-1 HE REPORTED IT TO MANAGEMENT. WE SENT HIM TO A DOCTOR. DOCTOR DID XRAY DID NOT FIND ANYTHING. HE CONTINUED TO WORK. DOCTOR SENT FOR MRI. MRI DETERMINED TORN LIGAMENT REQUIRING SURGERY.
