EMPLOYEE SURVEYING GRAVEL BARS, TICK AFFIXED ITSELF TO SKIN OF EMPLOYEE AND INFLICTED BITE IN UPPER CHEST, COLLAR-BONE AREA.
EMPLOYEE'S FOOT SLIPPED WHILE CLIMBING DOWN SCAFFOLDING AT EXCAVATOR CAUSING HIM TO FALL ABOUT 5'; BARS OF SCAFFOLDING HIT CHEST AND LOWER LEFT LEG.

WHILE WORKING AS A TRUCK DRIVER HE APPLIED THE POWER TAKE OFF IN ORDER TO DUMP THE LOAD OF GRAVEL IN THE TRUCK. HIS HAND SLIPPED OFF OF THE HANDLE AND HIT A PIECE OF METAL STICKING OUT OF THE SEAT OF THE TRUCK IN THE CAB, CAUSING PAIN IN HIS LEFT WRIST, THUMB, AND HAND.
WHILE STANDING ON THE BACK OF A BOTTOM DUMP TRUCK AND KICKING ROCKS OFF OF THE EDGE OF THE TRUCK TO PREVENT SUCH ROCKS FROM COMING OFF OF THE TRUCK ON THE HIGHWAY, EMPLOYEE SLIPPED WHEN GETTIN G READY TO CLIMB DOWN TO THE GROUND AND HIS FOOT GOT STUCK IN HOLE ON THE TRUCK AND HE HIT HIS SHIN, LOWER LEG ON THE TRUCK, CAUSING A SEVERE BRUISE.
