EMPLOYEE WAS CUTTING A PIECE OUT OF CONVEYOR BELT FORPLANT JOB. WHILE REPOSITIONING HIMSELF, KNIFE SLIPPED OUT OF BELT & CUT THE SIDE OF HIS RT. LEG, REQUIRING 10 STITCHES.
THE EMPLOYEE WAS INSTALLING AN IDENTIFICATION SIGN ON A FUEL TANK AND THE WRENCH HE WAS USING SLIPPED AND HIS WRIST HIT THE SIGN, RESULTING IN A LACERATION.

THE EE WAS CHANGING WELDING CYLINDERS. WHEN ONE OF THE CYLINDERS BEGAN TO FALL, EE ATTEMPTED TO CATCH IT. THE CYLINDER CRUSHED HIS FINGER AGAINST THE CART CAUSING IT TO BREAK.
EMPLOYEE WAS MOVING AIR TRACK DRILL WHEN AIR PIPE CAME LOOSE CAUSING WHIPPING ACTION. SAFETY CHAIN WAS IN PLACE. EMPLOYEE RECEIVED FRACTURE TO SHALL BONES IN LEFT LEG.
