conveyer belt had stopped on plant employee stopped and got out of loader to check belt. Rock fell off and hit employee in the face.

EE was cleaning by plant when material slide began sliding and hit EE in the shoulder causing injury, was taken to ER. Plant was shut down, then reinforced the material slide with anchor bolts and chains.
