Repair cone crusher and putting toggle plate in place, employee removed safety chain causing plate to slide down, hitting jack and pinching employee arm causing a compound fracture of right forearm. Both bones in forearm fractured

EMPLOYEE WAS TRYING TO CLEAN A TAIL PULLEY WHILE THE CONVEYOR WAS OPERATING. EMPLOYEE STUCK A TWELVE INCH CRESCENT WRENCH INTO A FIVE INCH OPERNING TO CLEAN SAND OFF OF THE END OF THE DISCHAR E CONVEYOR TAIL PULLEY.
