MARK WAS GREASING THE DRAGLINE DURING LUNCH TIME. HE WAS STANDING ON THE GEAR CASE GREASING THE FRICTION WHEN HE SLIPPED AND FELL, HITTING HIS HEAD ON AN ANGLE-IRON BRACE. THIS RESULTED IN A L ACERATION OF THE RIGHT SIDE FOREHEAD AND THREE STITCHES AS TREATMENT.

THE WEATHER WAS GETTING VERY BAD (IE FREEZING RAIN) WHILE BACKING UP TO THE DUMP SITE HIS FRONT WHEELS STARTED TO SLIDE THIS CAUSED HIS RT REAR WHEELS TO GO OVER THE SAFETY BERM OF THE DUMP C AUSING THE TRUCK TO FALL OFF THE DUMP SITGE INTO THE PIT SCOTT JUMPED FROM OPERATORS CAB LANDING ON HIS RT FOOT AND THEN FALLING TO THE GROUND ON HIS BACK
