EE was stacking bags and was breathing hard, taking in more air than the other hands when he passed out. He came to shortly and was rushed to the town clinic and then transported to the hospital by ambulance where it was confirmed he had high levels of carbon monoxide.

EE was climbing ladder and fell back on his rear end when slipping on the second rung. His arms flew back and he contacted a protruding bolt on an elevator with his left forearm, causing a moderately smooth 3 inch long cut. He went to Wyoming Medical Center to have it stitched and returned to work to finish his shift by his own choice.

A visit to doctor on 09/25/2009 revealed a strained tendon and nerve as it was pushed across a bone spur already present at this site. EE was put on light duty until October 1, 2009. No days of lost work are expected.

Employee was driving forklift down a hill on uneven dirt road at too high a speed, started to turn right and centrifugal force caused machine to fall over on left side. Seat belt was not in use and he was thrown from machine with left leg extended and protective cage landed on it a few inches above ankle, resulting in compound fracture.Surgeries required to reconstruct leg.

Employee was stacking 50 lb bags onto pallet when he felt a sharp pain in left arm and shoulder. Doctor believes it is a rotator cuff injury and may need surgery. No lost time as yet. Edoc 154045 corrects the date of injury to 01/04/2005 from 01/01/2005. Surgery was eventually performed on 02/11/2005: 5 days lost time and on light duty until spring - update will follow.

Employee exited loader, failed to use the bottom step and heard and felt a loud pop in right knee. This caused him to fall down and to do some limping for awhile. A visit to the doctor the next day indicated he compressed the cartilage.

THE EMPLOYEE TWISTED LEFT KNEE WHILE STEPPING DOWN FROM A FRONT-END LOADER. THE KNEE SWELLED, WAS OBSERVED FOR A PERIOD OF TIME, AND SUBSEQUENTLY OPERATED ON FOR A TORN CARTILAGE ON DECEMBER 1 6, 2003. HE WAS PLANT OPERATOR AT TIME OF ACCIDENT AND PLANT MANAGER AT TIME OF SURGERY.

HE TRACED THE SMOLDERING LIGNITE TO THE DUST COLLECTO R & WATERED DOWN THE HOT SPOT & THE FLOOR BENEATH IT. VARIOUS COMMUNICATIONS WERE MADE & THE FIRE DEPT HOSED DOWN THE COLLECTOR FROM 1:30-2:00 PM. CAUSE OF FIRE HAS NOT BEEN DETERMINED. MSHA

EE ACCESSED TOP OF RAILCAR TO MOVE SPOUT INTO LAST COMPARTMENT WHEN HE SLIPPED ON THE LID AND FELL BACKWARD. THE RETRACTABLE LIFELINE ACTIVATED, STOPPING AND JERKING HIM SUDDENLY, WHICH INJURE D HIS NECK AND HAS CAUSED DISCOMFORT.
