While driving a truck which was being towed (mud), ee was struck in the forehead by a chain when it failed. EE recieved 3 cuts to his forehead requiring stitches.

During the beginning of May, employee injured his knee while jumping off of a vehicle to the ground. At the time he did not feel it was necessary to see a physician. Towards the end of July, he expressed concern that his knee was bothering him. He was sent by Cannon Resources on August 11th to Denver to see his primary care physician as requested.
