THE EMPLOYEE WAS ASSISTING IN HOISTING THE SLUSHER BUCKET OUT OF THE TRENCH BY OUNTING THREE SHEAVE BLOCKS AND THREADING A 1/2 INCH CABLE THROUGH BLOCKS TO A LOCOMOTIVE. THE EYE PAD AND CHAIN HOLDING THE BLOCKS BROKE FREE AND HIT EMPLOYEE IN THE LEFT SHOULDER AND THIGH.
I.NAME WAS ASSISTING A MECHANIC IN PRESSING A BEARING OFF OF A PUMP SHAFT. THE SHAFT SLIPPED FROM UNDER THE PRESS TILTING THE SHAFT AND PINCHING THE EMPLOYEES RIGHT THUMB AGAINST A PRESS BRACE
