While opening the front gate the associate inserted a bolt with washer used to keep the gate from swinging shut. Before the bolt was inserted completely, the gate sprung backwards pinching the associates finger between the gate and the washer on the bolt causing a laceration to one finger that required stitches.

While delivering material I realized that my truck was having issues. I called towing company who came & loaded the truck. Then took the truck to the quarry to unload the material. After unloading the material, I proceeded to step out of the truck only to discover that the wrecker had raised the truck further from the ground causing me to fall & injure myself.

Employee was trying to remove a piece of metal that he had cut from the old jaw structure. He was moving it with a digging bar. He turned at the waist and pulled up on the bar and felt his lower back pull. The metal was 4'x 6' x 3/4".

While reworking a steel screen feed box EE cut a steel plate loose without securing it and it turned over on his arm. The plate cracked the bone in his right arm just above the wrist.

Cleaning chunks of ammonium nitrate out of the hopper on a repump unit, cut right arm and hand on the edges of the hopper.

WORK WAS BEING PERFORMED ON C-5 HAND PULLEY SCRAPPER. SCRAPPER WAS ALLOWED TO FALL AND PINCH FINGER.

EMPLOYEE WAS CHANGING FROM BUCKET TO HAMMER. HE WAS TIGHTENING HOSE ON HAMMER WHEN THE WRENCH SLIPPED OFF HITTING HIM ON THE RIGHT WRIST.

EE WAS CARRYING 2 WOODEN BLOCKS. HIS FEET SLIPPED ON ICE AND HE FELL, CATCHING HIS FINGER UNDER THE TWO BLOCKS CRUSHING AND SLICING TIP OF FINGER.

EMPLOYEE WAS USING A CROWBAR TO CHIP CONCRETE OUT OF THE PUGMILL. THE BAR SLIPPED AND STRUCK HIM IN THE RIGHT EYE- TEMPLY AREA.

EMPLOYEE WAS IDSASSEMBLING A HYDRAULIC BODY LIFT ASSEMBLY ON A 35 TON DUMP TRUCK. HE WAS USING A PORTAPOWER WITH 4 11/2 SOCKETS AS SPACERS TO PUSH THE PIN OUT OF THE BUSHING. THE LIFT ASSEBLY WAS BEING HELD BY A STRAP CONNECTED TO THE WHEN ON THE CRANE OF A MECHANICS TRUCK TO HOLD IT SECURE. HE STUCK HIS FINGER INTO THE BUSHING TO RETREIVE THE LAST SOCKET WHEN THE LIFT ASSEMBLY MOV

WHILE UNLOADING CASING, A JOINT OF CASING FELL ON HIS FOOT. IT HIT ABOVE HIS STEEL SAFETY TOE.

EMPLOYEE WAS UNHOOKING A PIECE OF STEEL RFROM A LOADER BUCKET WHEN THE STEEL WAS UNHOOKED IT FELL ON EMPLOYEES FOOT

WHILE PLACING A SCREEN PLATE ON SCREEN IT SLIPPED & CUT THE END OF EMPLOYEES LITTLE FINGER OFF ON HIS RT HAND
