EE stepped on a muddy gravel area as he was walking over to pick up a hose that he had been usig to wash clean out under the el jay screen. as he did, his right knee went sideways and popped.

EE WAS SHOVELING OUT TAIL PULLEY UNDER FEED BELT OF CRUSHER & FELT SOMETHING GIVE IN HIS ELBOW. HE SAID IT DID NOT BOTHER HIM UNTIL THE NEXT MORNING. HE WENT TO ST. JOSEPH'S HOSPITAL & WAS TOL D HE HAS TENNIS ELBOW & A POSSIBLE FRACTURE OF THE LEFT ELBOW. HE THEN CALLED OUR OFFICE & LET US KNOW OF THE INJURY.

EMPLOYEE WAS CLEARING DEBRIS FROM THE JAW CHUTE WHEN A ROCK SMASHED HIS THUMB AND DROVE THE EDGE OF THE THUMB NAIL INTO THE THUMB. EMPLOYEE DIDNOT REPORT OCCURANCE, AS HE FELT IT WAS NOT A PR OBLEM AT THE TIME. THE INJURY SUBSEQUENTLY BECAME INFECTED AND REQUIRED MEDICAL TREATMENT ON 7/11/94.
