DIGGING OUT THE FROZEN MATERIAL FROM THE CONVEYOR BELT, THE TRACK SHOVEL GOT CAUGHT & PULLED THE HAND WITH IT. GLOVES WERE FROZEN & STUCK TO THE HANDLE OF THE TRACK SHOVEL.

GRAVEL WAS JAMMED IN THE CRUSHER. TRYING TO PULL IT APART THE CHAIN BROKE AND FLEW OUT OF THE CRUSHER HITTING HIS WRIST AND BREAKING IT.
