Employee was installing 2 inspection ports on 2 conveying belts. A gust of wind blew lime into both of his eyes. After flushing his left eye was burned.

Employee was checking rotary feeder in hydrate area to see if it was working. He reached his hand up to feel if it was turning and it grabbed his middle finger of his right hand, cutting the tip of his finger off.

Employee was working in the maintenance shop near a work bench. His razor knife was on the table with blade extended. He reached for object on the floor. While reaching his arm was still on the table on top of the knife. With the pressure put on the blade, it penetrated his inner forearm midway between the wrist and the elbow making a 3/4" cut. It required 4 stitches.

HE WAS CLEANING OUT LIME FROM THE CHUTE TO THE MIXER BOX WITH A STEEL BAR. THE MIXER WAS RUNNING TO KEEP THE PUMP FROM PLUGGING. HIS ATTENTION WAS DISTRACTED AND HE TURNED TO SEE WHAT WAS HAPPENING BEHIND HIM. AS HE TURNED HE LIFTED THE STEEL BAR IN HIS HANDS UP, WHICH PUT THE OTHER END IN THE MIXER BOX. THE MIXER PULLED THE BAR OUT OF HIS HAND AND SLAPPED IT CAUSING INJURY

EE WAS THE OPERATOR ON THE HYDRATOR, LIME HAD COLLECTED ON THE WATER RING ABOVE THE MIXER. EE TOOK A STEEL ROD 1/4 IN DIA X 15" LONG TO KNOCK THE LIME INTO THE MIXER. THE ROD HOOKED ON THE MIX ER PULLING HIS HAND INTO THE METAL BOX AND THE ROD CUT OFF HIS RIGHT INDEX FINGER AND BROKE BOTH BONES IN HIS WRIST. THEY HAD PUT IN METAL PLATE IN THE WRIST.

EE WAS CLIMBING BACKWARDS DOWN THE LADDER IN THE KILN, HE WAS ON THE SECOND RUNG. AS HE WAS MOVING HIS HAND TO THE NEXT RUNG, HIS FOOT SLIPPED OFF THE STEP. HIS HOLD WAS NOT TIGHT ENOUGH, WITH ONE HAND, TO HOLD HIM AND HE FELL ABOUT 5 FEET TO THE CATWALK BREAKING HIS ANKLE.

EE HAD RAISED THE BED OF HIS TRUCK AND WAS CHECKING THE BRAKE OIL, OTHER HIS SUPERVISOR WAS WALKING TOWARD THE TRUCK, HE WAS COMING DOWN THE LADDER TO TALK TO HIM. HE MISSED A STEP AND STARTED TO FALL, HE TRIED TO CATCH HIMSELF AND CAUGHT HIS ARM BETWEEN THE STEPS. THE WEIGHT FROM THE FALL CAUSED THE BREAK IN HIS ARM.

IT WAS RAINING LIGHTLY AND THE TOP OF THE TRAIN CAR WAS WET. LIME DUST MIXED WITH THE RAIN MADE THE SURFACE AREAS OUTSIDE OF THE CARS GRATED CATWALK, SLIPPERY. THE EMPLOYEE STEPPED OFF THE C ARS GRATED CATWLK, TO MOVE A DUST SHIELD WHERE UPON HE SLIPPED AND FELL 14' TO THE GROUND BELOW BREAKING THE HEEL BONES IN BOTH FEET.

EMPLOYEE WAS CLEANING LIMESTONE FROM THE HOPPER. THE MATERIAL THE EMPLOYEE WAS STANDING ON WHILE PERFORMING THIS TASK GAVE WAY, CAUSING THE EMPLOYEE TO FALL INTO THE HOPPER AND STRIKE HIS KNEE .
