Employee was tightening bolts on the rock hammer located at the primary crusher. He was using a 16 lbs hammer and a knock wrench to tighten bolts strained right forearm and elbow. He was placed on restrictive duty on 5/28/08 no lifting over 5 lbs.

EMPLOYEE'S WERE TRYING TO ALIGN THE OUTSIDE HOLEON THE SHOVEL TRACK WIHT THE TRACK LINK PIN. THEPIN HAD BACKED OUT OF THE FRONT LINK. AS THE TRACK WAS ROTATED TWO METAL SHARDS FLEW OFF THE PIN AND STRUCK THE EMPLOYEE IN THE NECK AND LEG.

EE WAS ATTEMPTING TO REMOVE THE BRAKE ADJUSTER IN FROM THE HOIST DRUM ON A NORTHWEST DROPBALL. WHILE LAYING ON HIS CHEST ACROSS THE GEAR BOX, HE BEGAN PULLING ON THE PIN. THE PRESSURE RESULTIN G FROM THIS ACTION CAUSED THE EE TO REINJURE THE CARTILIGE IN HIS RT RIBS. THIS IS THE 4TH OCCURRANCE OF THIS INJURY TO THIS EE.

EE WAS LIFTING WHEEL CHOCK TO PLACE IN RACK ON FRONT BUMPER OF TRUCK. WHEN EE ATTEMPTED TO PLACE CHOCK IN RACK, HE NOTICED PAIN IN LOWER BACK RIGHT SIDE.
