EE was attempting to remove hammer hose from a hydraulic hammer. Though the pressure had been released but as he was removing hose blew off and struck saftey glasses. Minor cut under eye but further exam revealed fracture on bone.

EMPLOYEE WAS ON GROUND GREASING LOADER BUCKET EMPLOYEE WAS BENDING OVER WHILE GREASING. WHEN HE STOOD UP HE BUMPED HIS BACK ON BUCKET ARM.
EMPLOYEE WAS STEPPING DOWN FROM EQUIP AND TWISTED RIGHT FOOT. DID NOT SEEK MED TREATMENT UNTIL 7-6-94. SURGERY TO REMOVE BONE CHIP.
EE CLAIMS HE WAS EXITING THE R-35 DUMP TRUCK WHEN HIS PANT LEG CAUGHT ON DUMP LEVER. EE STATS THAT HE LOST HIS BALANCE & FELL TO GROUND.
