Rinker Materials · Fall to the walkway or working surface

Area posted DANGER Do not enter unstable ground. Employee walking across to berm to retrieve rope that stabilizes float pump in retaining pond. Settling of the ground caused major cracks on top of berm. Employee heel on edge of crack as he attempted to step over it. Dirt gave way and he fell into the crack up to groin. Complained of pain in wrist, elbow, thigh and abdomin.

Operator was trying to turn off the main water supply at the crusher to change a valve. The operator was standing on top of pipe turning valve when his feet slipped. He fell into the hole that was dug under the pipe in order to gain access to the bolts that secure the pipe. He sprained left ankle - notified supervisor but continued to work a normal shift day.
