EMPLOYEE WAS MOVING GRAVEL FROM PRODUCTION AREA TO STOCKPILE AREA. AS HE BEGAN MOVING FORWARD WHILE LEVELING AND LOWERING HIS BUCKET HE LOOKED OUT HIS SIDE WINDOW TO CHECK ON ANOTHER PILE. HIS CUTTING EDGE STRUCK THE GROUND. ALTHOUGH HE WASWEARING HIS SEAT BELT HE STRUCK HIS HARD HAT AGAINST THE WINDSHIELD. THE HAT FORCED HIS SAFETY GLASSES FORWARD CUTTING HIS NOSE, REQUIRE 3 STITC

EMP WAS STRUCK ON RIGHT SIDE OF HIS FACE BY HANDLE OF WINCH WHICH HE WAS RELEASING. WINCH CABLE WAS UNDER TENSION. EMP SUFFERED A LACERATION TO RIGHT CORNER OF MOUTH AND LOST 2 TEETH AND BROKE ANOTHER TOOTH.
