EE WAS SPOTTING THE CRUSHER UNIT INTO PLACEMENT.HE WAS TO THE SIDE & HE STEPPED INTO CHECK THE ALIGNMENT OF THE CRUSHER UNITS. HE STAYED THERE WHILE THE UNIT WAS BEING BACKED INTO PLACE. THE O THER SPOTTER WAS NOT TOLD BY EE TO STOP THE CRUSHER UNIT THAT WAS BEING BACKED INTO PLACE NOR DID HE STEP OUT OF THE WAY. HE WAS CRUSHED BETWEENTHE TWO UNITS & DIED MOMENTS LATER.

EMPLOYEE WAS DISENGAGING ROCK, FROM PULLEY WITH METAL BAR. LOADER PUSHED ELECTRIAL CORD INTO CRUSHER TRAILER CAUSING IT ARCH AND BREAK THE GROUND CABLE WHICH IN TURN EMPLOYEE BECAME THE GROUN D.

EE HAD SHUT THE MOTOR OFF THE BELT TO DISLODGE A ROCK OUT FROM THE TAIL PULLEY. SHE HAD HER HAND AROUND THE HEAD PULLEY BELT TRYING TO PULL TO RELIEVE THE PRESSURE OFF THE ROCK. WHEN THE ROCK DISLODGED, THE FORCE OF THE BELT BEING RELEASED CAUSED THE HEAD PULLEY TO TURN, WHICH CAUGHT HER FINGER.
