ACCIDENT OCCURRED AS SUPPLY TRUCK DRIVER FOR UNITED SUPPLY CO WAS TRANSPOR TING SUPPLIES TO MINE ON ICE & SNOW COVERED ACCESS ROAD. TRUCK STALLED &/OR LOST TRACTION WHICH RESULTED IN TRUCK SKIDDING APPROX 90' BACKWARD. EMPL EXITED TRUCK & WAS STRUCK & TRAPPED BY LEF

EE WAS TRANSPORTING COAL FROM FACE WHEN COAL OR ROCK FELL FROM SHUTTLE CAR ON HIS FOOT. THE EE WORKER THE SHIFT AND CALLED TO REPORT ACCIDENT ON 1-29-90 HE DID TELL THE FOREMAN HE HAD HURT HIS FOOT BUT WAS OK.
