ON 12/19/89 AT APPROX 4:30 PM A FATAL HAULAGE ACCIDENT OCCURRED ON ACCESS ROAD TO KOCH CARBON INC HALL BRANCH 1 MINE. ACCIDENT OCCURRED AS SUPPLY TRUCK DRIVER FOR UNITED SUPPLY CO WAS TRANSPOR TING SUPPLIES TO MINE ON ICE & SNOW COVERED ACCESS ROAD. TRUCK STALLED &/OR LOST TRACTION WHICH RESULTED IN TRUCK SKIDDING APPROX 90' BACKWARD. EMPL EXITED TRUCK & WAS STRUCK & TRAPPED BY LEF
