Mechanic was hit by metal shard while replacing a bucket tooth on a loader.

Employee was stepping down from the loader and missed the last step resulting in shoulder and arm strain. Employee underwent surgery on 11/17/21, making this a Lost Time accident

Employee was found unconscious near the plant control building. The incident appears to be the result of a personal medical condition. Preliminary medical information indicates this is a non-work related event.

Employee was assisting mechanic replacing springs on a Mack Haul Truck. When all repairs were complete, employee was lowering Mack Truck with rachet type jack when the handle raised quickly and pinched employee's left index finger between the bed and the handle. Mine foreman took employee to hospital where he received x-rays and 8 stitches to left index finger.

Employee raised shop overhead door manually and strained muscle in mid to lower back area.

Employee was attempting to turn the bowl on the Standard crusher at the Tertiary when one of the pull lugs on the bowl broke off. It struck the employee in the leg just below the boot line causing a fracture in the Tibula.

Employee was dislodging material stuck in a discharge chute when his right hand got pinched between the shaker deck and the chute. Medical treatment consisted of a rigid splint and pain pills. He was on light duty for 2 days and no follow up visits were required. His injury was a bruise to the top of his right hand.

Employee was exiting Mack twin screw haul truck and fell when his footing slipped on fuel tank. As the employee fell his left elbow was cut from hitting the rocker panel. Employee was taken to Doctor where his elbow cut was closed with glue instead of suture. Employee returned to work with no restrictions.

Employee exiting truck tripped over wood block and fell on elbow.

WHILE FABRICATING A CHUTE, A PIECE OF STEEL PLATE FELL OUT OF POSITION AND STRUCK EE'S FOOT, RESULTING IN FRACTURE TO THE METATARSAL REGION.

EE WAS LIFTING A LEAF SPRING & OVEREXTENDED HIS ABDOMEN AREA AND IS GOING TO HAVE A HERNAI REPAIR ON 6/8/99

STRAINED LOWER BACK WHILE LOADING A SHOT.

A LARGE ROCK WAS HUNG IN THE JAW CRUSHER. THE MEN WERE TRYING TO REMOVE IT. THE BAR THEY WERE USING TO PRY WITH SLIPPED AND KNOCKED GLENS SAFETY GLASSES OFF WHEN THE GLASSES CAME OFF THEY HIT HIS FACE AND CUT HIS HEAD OVER THE RIGHT EYE. THE DR. CLEANED THE CUT AND THE CUT REQUIRED 3 STITCHES.

EE WAS CUTTING AN EMPTY HEET BAG OPEN, WHEN THE KNIFE SLIPPED.

EMPL REMOVING LIGHT FIXTURE.IT SLIPPED AND CUT THUMB.

THE INJURED WAS3HELPING REPLACE GASKET AROUND THE CRUSHER DOOR.THE CHAIN HOIST HOOK CAME OFF THE DOOR LETTING IT FALL.THE DOOR HIT THE INJURED ON THE LEG KNOCKING HIM BACK. A GRATE BAR CAME OU T OF THE CRUSH9R,HIT HIM ON THE FOREHEAD,OVER THE LEFT EYE.

I NAME WAS WELD3NG ON A SHAKER ABOVE HIS HEAD HE SAID HE HAD HIS WELDING HELMET ON A PIECE OF STEEL FELL IN HIS EYE PENETRATING THE STROMAL OF LEFT EYE HE BELIEVED IT WAS A FLASH BURN BUT WENT TO DR ON 9 18 86 THEY REMOVED THE STEEL ON THE 19TH USING A SPUD WASHED IT OUT PRESCRIBED A MEDICATION NO DAMAGE TO EYE
