CONDITIONS WERE DRY, AND NIC SUNNY DAY. EE, CLIMBING DOWN LADDER AFTER CLEANING THE DECK OF THE CONE. HE PUT HIS RIGHT FOOT ON THE GROUND, THEN FOLLOWED BY LIFTING HIS LEFT FOOT TO PUT THEM BO TH FLAT ON GROUND WHEN HIS KNEE GAVE OUT (RIGHT) FROM HIS OWN BODY WEIGHT. HE THEN COLLAPSED TO GROUND AT THE BOTTOM OF LADDER. THE LANDING WAS CLEAR W/O ANY TRIPPING HAZARDS.

EMPLOYEE SAYS HE ACTED CARELESS AT THE END OF THE DAY. HE SAW SOME ROOT STICKING OUT OF ROLLER AREA IN CONVEYOR, THEN REMOVED GUARD AND REACHED IN TO PULL ROOTS OUT BUT ROLLER GRABBED HIS LITT LE FINGER AND IT WENT BETWEEN THE BELT AND ROLLER.
