While resetting a scraper on conveyor #2 the bar slipped on the shaft and the employee's right index finger was caught between the scraper counter weight bar and conveyor frame. This caused a fracture between the 2nd and 3rd knuckle.

While placing screen deck in position it slipped & pinched employee's ankle.
Employee was helping co-worker after a screen deck pinched coworker's ankle and EE slipped falling on knee.

A loader operator was closing the door on Volvo Front End Loader. When ee shut the door, ee pinched the tip of right middle finger between the loader door and the door frame. This resulted in a laceration that required sutures.

While dumping a load of rock in the primary crusher, a rock fell over the side of the haul truck dump bed in front of the left rear tire. The employee attempted to manually slide the rock out of the way when he stated his right elbow popped. Employee reported the incident immediately on 6/9/14, but did not want medical treatment until 6/12/14.

Employee changing oil in Volvo A25C haul truck. Belly pan got wedged by a jack. Employee attempted to manually lift the belly pan and felt a sharp pain and burning sensation in lower right side. Incident occurred on 5/15/13, first treatment on 6/5/13.
Employee was connecting a water line for the dust suppression system to the water truck. A gust of wind blew dust into the employee's left eye resulting in a scratch. Employee was wearing prescription safety glasses with side shields. Prescription eye drops were issued.

Employee was preparing to repair a belt splice. The conveyor belt was in a bind so to relieve the tension on the belt, he and a co-worker used utility knives to cut the "Dirthog" belt scraper. The utility knife slipped and cut employee's hand between his left index finger and thumb.
