EMPLOYEE REPORTED TO WORK AT 0700, THE MORNING OF THE ACCIDENT. HE PERFORMED HIS NORMAL DUTIES AS A GENERL HELPER UNTIL ACCIDENT. AT 1730, HE WAS CLEANING BELOW THE #1 BELT WITH A WATER HOSE. WHEN THE TWO BOLTS HOLDING THE POWER CHUCK PLATEBROKE ON THE OFF-DRIVE SIDE OF CRUSHER, THE PLATE HIT THE CONVEYOR FRAME AND THEN FELL ONTO EMPLOYEE'S RIGHT ANKLE.
