EE hammered a pin and a sliver of metal hit his right eye. He went home, his wife took him to a clinic. Dr. removed the sliver. He had two follow up appt. with Dr., Dr. made ee an appt. with another Dr. (Cornea specialist) 3/12/2014 in L.A.

EE had discomfort in left shoulder. He went to doctor 3/8/13 to have it looked at. EE informed us that he went to the doctor and had a problem with his shoulder on 4-25-13. He was put on light duty.

Employee was working in the mechanic area when he received a superficial cut on his right hand. The cut was cleaned and bandaged. Employee said he was fine to continue working. That night his hand began to swell so he went to emergency room where they treated him for infection and gave antibiotics.
