During operator pre-shift inspection operator added coolant to dozer. After operator added coolant the radiator cap was not installed on properly. Operator at 1:00 p.m. noticed cap was on wrong. Operator shut off engine and tried to loosen radiator cap which resulted in hot coolant to be released.

MECHANIC WAS WORKING ON HEAVY EQUIPMENT, BENT OVER THE ENGINE TO TIGHTEN DOWN A BOLT (VERY TIGHTSPOT - AWKWARD POSITION). WHEN STANDING BACK UP HE BUMPED HIS HEAD AGAINST A BOLT/SCREW PROTRUDI NG FROM THE EQUIPMENT CAUSING A GASH TO THE TOP OF HIS HEAD REQUIRING STITCHES.
