Kilauea Crushers Inc · Fall from machine

Was checking air filter on Volvo 350 loader while standing on tire and lost balance and fell. Twisted while falling to avoid hitting head and landed on arm.

Kilauea Crushers Inc · Struck by rolling or sliding object

While cleaning debris from the grizzly feeder he lost his balance and slid down a chute about 39" onto a pile of rock. One of the rocks rolled down and bruised his right leg. This machine was shut down and locked out at the time the task was being performed.
