EE slipped off the hood at the front of the dozer, possible because of morning dew. His feet went out from under him and he thinks he hit everything he possible could on the way down. He ended up curled under the dozer. He got up before anyone got to him and crawled out from underneath the dozer.

He had climbed off the excavator and walked to his truck to get something to drink. He walked back to the excavator and as he was climbing back into his cab, he felt severe pain in his lower back. He saw a dr in the afternoon and came back to work the next day. Since then it has just go on worse and he has to have surgery.

Inj EE & another EE were changing eng oil in D11N dozer. Due to weather condition, equip was very slippery. Inj EE lost his footing on catwalk, holding on to hand rail at door post, body weight jerked hand loose, pulling & straining rt shoulder & fell onto track, push arm and onto ground. Not sure which hit caused lower back injury.

EE was driving Euclid 85 Ton truck up the ramp to the dump area. Conditions of the ramp had been changing through the day. Truck lost traction & started sliding backwards. EE turned truck into bank on driver's side to avoid hitting another truck. Truck turned over. EE climbed out of the cab & jumped 10' to ground. He slipped, landing on his right shoulder resulting in a fracture.

AFTER REPLACING THE FAN BELT ON THE MILLER BIG 40 WELDER, EE DECIDED TO ADJUST THE HIGH IDLE SPEED BEFORE PUTTING THE GUARD BACK OVER THE FAN. WHEN HE TURNED THE THROFFLE, THE COTTON GLOVE HE WAS WEARING GOT CAUGHT & PULLED HIS FINGERS INTO THE FAN, CUTTING 3 FINGERS.

HE FLIPPED OVER BACKWARDS & LANDED ON HIS RIGHT SHOULDER & THEN ONTO HIS BACK, CAUSING A SHOULDER SEPARATION, NECK SPRAIN W/SPASMS

TWO EES WERE DISCONNECTING THE DISCHARGE HOSE FROM THE WATER PUMP, AND FLUSHING IT TO DISCHARGE THE ICE BUILDUP FROM THE NIGHT BEFORE. AS IT WAS BEING PUT BACK TOGETHER, EE SLIPPED AND FELL O N THE FROZEN GROUND, INJURING HIS LOWER BACK. THERE ARE TWO HERNIATED DISKS IN HIS LOWER BACK.

EE AND COWORKER HAD JUST SET A TRAVEL SHAFT AND SPROCKET DOWN UNDER THE DRAGLINE JUST OUTSIDE THE CAR BODY WITH THE CRAN ON EE'S MECHANIC TRUCK. EE UNHOOKED THE CABLE AND WAS GOIDING THE HOOK BACK OUT WHEN THE CABLE GOT CAUGHT ON A BOLT ANDPULLED HIM OFF BALANCE. HE JUMPED OVER THE SHAFT AND TRIED TO DUCK UNDER THE CAR BODY TO REGAIN HIS FOOTING WHEN HIS HEAD HIT THE CAR BODY.
