Employee was using a pry bar to put tracks on D11 Dozer when he pulled he hears a loud noise in his left elbow and had immediate pain. Torn left Bicep.

Multiple Physical Lower Back injury due to repetitive motion on a Cat D11N dozer.

Fair Weather. Mechanic welder changing brakes on a Cat 5130B Hydraulic Excavator when he sprained his back.

Employee was getting oil for equipment when he slipped on the ice, he caught himself on the tailgate of his truck but he twisted his lower back & hip.

Was walking from shop area to the fuel truck to start it up, area was icy causing the employee to fall onto his back and injuring his left arm.

Working on grease bucket on Cat 5130 B Hydraulic Excavator when he slipped on tracks & caught himself with his left arm. Causing Injury.

Lifting & Rolling out 450 pounds of Water Discharge Hose. Caused Torn Muscle to Lower Abdomen.

Mechanical work resulting in Chronic Bilateral Median Mononeuropathies at the Wrist. First saw Doctor on 12/14/2012. Did not miss work till 3/14/13 when he had surgery on wrist.

Mechanic was working on dozer blade, he had a ladder against the dozer. The wind blew the ladder over causing head cut and blunt head trauma.

Was pulling rock out with dozer when dozer slipped and jarred operating in the cab.

Lifting bouncing in rock truck, jarring ,sprain, strained and ruptured disk in back.

Using a pry bar to loosen and turn out a nut on the D11 Dozer track frame.EE worked until 7/10/12 having surgery on shoulder.

Fueling D11R Dozer when he slipped, the employee caught himself and strained his shoulder.

Doing mechanic work on loader bucket. Lifting plate onto bottom of bucket of loader with other mechanics help. Pulled his back, pain occurred immediately.

Was operating 345B cut excavator to pull brick rock lose and the bucket slipped off the rock jarring him back and forth causing whip lash motion causing injury to upper back area and rib cage.

EE slipped off the hood at the front of the dozer, possible because of morning dew. His feet went out from under him and he thinks he hit everything he possible could on the way down. He ended up curled under the dozer. He got up before anyone got to him and crawled out from underneath the dozer.

Mechanic was changing ripper tooth on D11 dozer. When he went to lift the ripper tooth up, he had immediate pain in his lower back.

There was rain that froze, the job site was slippery. The employee was leaving his equipment going to his vehicle to go home when he fell on ice and broke his ankle.

EE was working off the back on his mechanic truck, he stepped aside and stepped on cribbing block and twisted his left knee.

He had climbed off the excavator and walked to his truck to get something to drink. He walked back to the excavator and as he was climbing back into his cab, he felt severe pain in his lower back. He saw a dr in the afternoon and came back to work the next day. Since then it has just go on worse and he has to have surgery.

Inj EE & another EE were changing eng oil in D11N dozer. Due to weather condition, equip was very slippery. Inj EE lost his footing on catwalk, holding on to hand rail at door post, body weight jerked hand loose, pulling & straining rt shoulder & fell onto track, push arm and onto ground. Not sure which hit caused lower back injury.

EE was driving Euclid 85 Ton truck up the ramp to the dump area. Conditions of the ramp had been changing through the day. Truck lost traction & started sliding backwards. EE turned truck into bank on driver's side to avoid hitting another truck. Truck turned over. EE climbed out of the cab & jumped 10' to ground. He slipped, landing on his right shoulder resulting in a fracture.

Mechanic was tightening the alternator belt on the Kawasaki Z95 Loader using a pry bar when the pry bar slipped and struck him in the head above the left eye causing lacerations.

Heavy equipment operator was walking the hill on the job to see where they wanted to make the next cut with the equipment. When they were going down the hill the injured employee caught his toe in a root & fell on his face.

Drives Cat 773 Rock Truck and by repetitive motion and dumping, jarring strain/injury to lower back.

CAHNGING AIR FILTERS IN THE D10N DOZER. WHILE LIFTING THE NEW FILTER UP TO INSTALL IT, HE SPRAINED HIS BACK. FILTER WEIGHTED APPROX 20-25 LBS

AFTER REPLACING THE FAN BELT ON THE MILLER BIG 40 WELDER, EE DECIDED TO ADJUST THE HIGH IDLE SPEED BEFORE PUTTING THE GUARD BACK OVER THE FAN. WHEN HE TURNED THE THROFFLE, THE COTTON GLOVE HE WAS WEARING GOT CAUGHT & PULLED HIS FINGERS INTO THE FAN, CUTTING 3 FINGERS.

EE'S WERE WALKING BACK TO THEIR CARS AT QUITTINGTIME. THEY HAD JUST CROSSED A COLLECTION DITCH &STARTED DOWN A SMALL EMBANKMENT WHEN INJURED TRIPPED AND FELL. HIS LT. LEG BUCKLED UNDER HIM, CA USING THE LIGAMENTS AND/OR MUSCLES IN HIS LT. THIGH TO TEAR.

EE AND THE MECHANIC WAS CHANGING OIL IN THE D11 DOZER. THEY WERE BOTH ON AND OFF THE DOZER SEVERAL TIMES. AS HE STEPPED OFF THE DOZER HIS FOOT SLIPPED OFF A ROCK, JAMMING HIS TOES INTO THE GRO UND, AND CAUSING HIM TO FALL.

EMPLOYEE WAS GREASING THE CONVEYOR WHILE THE AUGER MACHING WAS BEING SET UP FOR THE NEXT HOLE. APPARENTLY HID LGOVE GOT CAUGHT IN THE MOVING BELT PULLING HIS HAND IN BREAKING THE RING FINGER O N HIS LEFT HAND.

EE HAD BROUGHT 41B DOZER OUT OF PIT TO GET SOME OIL. AFTER PUTTING OIL IN DOZER, HE WAS CLIMBINGDOWN OFF DOZER WITH OILCAN IN HIS HAND. HE WAS FACING DOZER, STEPPING DOWN FROM TRACK TO PUSH AR M, WHEN HIS FOOT SLIPPED DOWN BETWEEN PUSH ARM &TRACK FRAME. HE FLIPPED OVER BACKWARDS & LANDED ON HIS RIGHT SHOULDER & THEN ONTO HIS BACK, CAUSING A SHOULDER SEPARATION, NECK SPRAIN W/SPASMS

EE WAS STEAM CLEANING THE ROCK TRUCK, STANDING ON THE HYDRAULIC TANK APPROX 6' OFF THE GROUND, WHEN HE SLIPPED AND FELL OFF BACKWARDS. HE LANDED ON THE GROUND ON HIS BUTTOCKS. HE CONTINUED WOR KING AND BY THE END OF THE DAY HIS LOWER BACK WAS GETTING SORE. HE WORKED UNTIL ABOUT 0900 FRIDAY AND THEN DECIDED THAT HE SHOULD GO TO THE DR.

TWO EES WERE DISCONNECTING THE DISCHARGE HOSE FROM THE WATER PUMP, AND FLUSHING IT TO DISCHARGE THE ICE BUILDUP FROM THE NIGHT BEFORE. AS IT WAS BEING PUT BACK TOGETHER, EE SLIPPED AND FELL O N THE FROZEN GROUND, INJURING HIS LOWER BACK. THERE ARE TWO HERNIATED DISKS IN HIS LOWER BACK.

EE AND COWORKER HAD JUST SET A TRAVEL SHAFT AND SPROCKET DOWN UNDER THE DRAGLINE JUST OUTSIDE THE CAR BODY WITH THE CRAN ON EE'S MECHANIC TRUCK. EE UNHOOKED THE CABLE AND WAS GOIDING THE HOOK BACK OUT WHEN THE CABLE GOT CAUGHT ON A BOLT ANDPULLED HIM OFF BALANCE. HE JUMPED OVER THE SHAFT AND TRIED TO DUCK UNDER THE CAR BODY TO REGAIN HIS FOOTING WHEN HIS HEAD HIT THE CAR BODY.

THE FOREMAN WASBOARDING THE DRAGLINE IN WET AND MUDDY CONDITIONS. AS HE CLIMBED UP ON THE TRACKS, HIS RIGHT FOOT SLIPPED BETWEEN TWO OF THE PADS AND HE LOST HIS BALANCE AND FELL BACKWARDS. TH E MUSCLES IN HIS CALF AND SHIN WERE BRUISED AND SPRAINED.

DOZER OPERATOR WAS CLEANING THE TRACKS AT THE END OF HIS SHIFT IT HAD BEEN RAINING HEAVILY & IT WAS VERY MUDDY AS HE WAS SHOVELING THE MUD FROM AROUND THE TRACKS & ROLLERS HE SLIPPED A& FELL F LAT ON HIS BACK FALL RESULTED IN CONSIDERABLE LOWER BACK PAIN TO WHICH POINT HE WAS NOT ABLE TO WALK ON HIS OWN

THE DOZER OPERATOR WAS FUELING HIS DOZER WITH THE DOZER RUNNING APPARENTLY THE OIL COOLER FAN BLEW THE FUMES FROM FUEL OIL INTO HIS FACE HIS NOSE BEGAN TO BLEED A SHOR WHILE LATER
